THE LID OVER THE REEL COMPARTMENT ON THE SHUTTLE CAR VIBRATED SHUT, WHILE THE BUGGY WAS BEING MOVED.
HE WAS PUSHING COAL WAGON INTO TIPPLE HE WAS USING PRY BAR AND IT SLIPPED WHILE HE WAS PRYING UP ON THE WAGON STRAIN TO LOWER BACK.
THE CLEW WAS PULLING MINEN CABLE UP. EMPLOYEE TWISTED BACK, WHEN CABLE GOT TIGHT.

INJURED WAS TIEING UP CABLE. STANDING IN BOOM OF S.C. LEAPED OFF OF S.C. INTO A HOLE IN TRAM WAY-CAUSED BY S.C. TIRES. EMPLOYEE TWISTED RT. KNEE SENT TO MT. PLEASANT HOSPITAL-FOUND KNEE IN BAC K TO HAVE SOME BROKEN BONES.
EMPLOYEES HAVE BEEN USING ACT/OXY TANKS-EVERYTHING WAS TURNED OFF AND IN A SECURE POSITION-FOR SOME REASON CAUGHT ON FIRE.NO PERSON IN SHOP AREA AT TIME.FIRE DEPT CALLED-THEIR INVESTIGATION,UN CERTAIN WHAT CAUSED IT.
INJURED WAS SLEDGING A PIECE OF ROCK THAT CAME DOWN IN SC TRAM ROAD, RIB ROLLED OUT STRIKING EMPLOYEE ON THE BACK SIDE, TOWARDS BUTTOCK AREA
SECTION WAS IN 3 WAY INTERSECTION OF GOB BOTTOM PRESSURE. HAD STARTED ALL MACHINERY MOVED BACK FOR WEEKEND. PRESSURE DUMPED 200 0UTBY FACE AREA.
A COAL WAGON OFF TRACK- JACKING CAR UP WITH SLATE BAR. WAGON KICKED OUT. SLATE BAR CAME ACROSS THE RIGHT EYE BROW CAUSING A LACERATION. EMPLOYEE SENT TO SOUTH HILL EMERGENCY ROOM NEEDING 7 STI TCHES.
PIECE OF RIB ROLLED OUT STRICKING VICTIM ON REAR OF LEFT LEG. SENT TO HOSPTIAL TO BE CHECKED.

THE ROOF CONDITIONS HAD SEVERAL CLAY VEINS AND CUTTERS ROOF CONTROL PLAN WAS BEING COMPLIED WITH THE LAST LFIFT BEFORE PUSH OUT STUMP WAS BEING TAKEN WHEN INTERSECTION STATRED WORKING. ROCK C AUGHT INER HEAD AND COVERED TO IN FRONT OF CANOPY.

FALL ON MAIN HAULAGE. LIP OF A OLD EXISTING FALL- FALL APPROX. 30 FEET LONG, 4 TO 6 FEET HIGH.POWER REMOVED FROM FALL AREA. FALL WILL BE BOLTED AND LOADED AS TO PLAN OF MINE FOREMAN.
FALL ON MAIN HAULAGE OUTBY 2 STRAIGHT AND MIDOLE SWITCH FO 01D HILTON. FALL APPROX. 55 FEET LONG 8 TO 10 FEET OF ROOF CAME DOWN. BOTH ENTRANCES. DANGERED OFF. POWER REMOVED FROM FALL AREA AND RE-ROUSED.

A SECTION OG RO2F APPROX 100' 15' FELL ON THE TRACK HAULAGE RD AT THE 1ST BLOCK WALL ON #1 TRACK. THE CAUSE OF THE FALL WAS LOOSE UNCONSOLIDATED SHALE SEPERATING ABOVE THE ANCHORAGE ZONE OF TH E 4' BOLTS. THE3HAULAGE WAS DEVELOPED IN 1885 & HAD FALL SEVERAL TIMES IN THE YEARS FOLLOWING NUMBEROUS CLAY VEINS & PARTINGS HAVE WEATHER CAUSING THE COLLAPSE.
I.NAME OBSERVIN1 LOADING ROCK ON HAULAGE WHEN STEEL CABLE FOR MOVEMENT SIDE TO SIDE OF DISCHARGE BOOM BROKE.OPERATOR AND I.NAME PUSHED BOOM TO SIDE SO THEY COULD LOAD ROCK BY HAND UNTIL REPAIR ED HOWEVER,BOOM3SWUNG BACK INSELF PUSHING I.NAME CAUSING HIM TO BUMP ELBOW AGAINST COAL RIB.
INJURED WAS CHE7KING THE 440 VAC TRANSFORMER WUTH A METER WHEN PHASES ON THE TRANSFORMER WENT TO GROUND CAUISNG A THERMAL FLASH.

THE LEFT PILLAR SPLIT OF A RETREAT SECTION HAD TO BE MINED OUT OF NORMAL SEQUENCE BECAUSE OF ROOF CONDITIONS. TO HAVE A GOOD FALL 1 BLOCK OF COAL WAS MINED TO MANY, BEFORE RETRETATING THE RIGH T SIDE. AFTER THE FALL ON THE LEFT IT APPEARED SAFE AND QUITE TO MINE THE RIGHT. HOWEVER IN THE PROCESS OF CLEANING THE RIGHT THE INTERSECTION FELL.
VICTIM TIGHTENING UP GATHERING ARM BOLT ON 486 LN MINER.WRENCH SLIPPED AND VICTIM HIT ELBOW ON CONVEYOR PAN.
SETTING POST IN FACE AREA OF RETREAT SECTION 45/BUTT.ROOF COAL GAVE LOOSE AND FELL DOWN ACROSS I.NAME HEAD AND DROVE HIM TO GROUND.
VICTIM WAS REPLACING THE TRAM MOTOR ON A 486 L.N. CONTINUOUS MINER. HE HADTHE BAD ONE LOOSE AND HAD TO PRY IT OUT OF THE MACHINE. WHILE LAYING ON HIS BACK HE DID SO BUT WAS UNABLE TO MOVE OUT
VICTIM WORKING AS MOTORMAN AND MECHANIC STEPPED OFF LOCOMOTIVE ONTO A LUMP OF COAL AND TWISTED HIS BACK.
VICTIM WAS OPERATING A LOCOMOTIVE PULLING LOADS FROM THE 45 MAINS SECTION WHEN A PIECE OF SLATE FELL FROM THE ROOF AND HIT HIS RIGHT WRIST. THE INJURY WHICH OCCURRED WAS MAINLY CONTUSIONS OF T THE RIGHT WRIST.
