He was checking a water pump hose for frozen spots when he tripped on a piece of frozen dirt and fell face first to the ground. He caught himself when he was falling with both hands breaking both wrists.

He was tightening bolts on a Cat D11 dozer when the wrench slipped and struck him on his wrist and fractured it.

Accident type, without injuries

Truck owned LJF Inc.enroute for load of coal-Bonita Job. Truck shut off going up grade, coasted truck backward to side road. Driver saw electrical smoke coming between front of battery box & rear of front tire. Wires started flaming, put out with fire extinquisher, flames kept erupting, fire out of control. Loader operator got water in his bucket dumped on fire until out.

He was moving a 1000 pound piece of steel when the cables lost tension and it fell onto his right leg. Broken lower right leg.

While operating rubber tired front end loader in rock in pit area the tooth of the loader slipped off a rock which shook the loader and he felt pain in his lower back.

Hydraulic line broke on Hitachi 1800 Shovel, employee slipped on oil and fell.

While climbing up the ladder on Cat 980C Wheel Loader, he lost his grip and footing on ladder and fell to the ground injuring his chest and leg.

He was walking from his pickup truck to the wheel loader to start work when he slipped on some ice that sloped downward. He lost his footing and rolled his ankle and then fell down on it, spraining his ankle.

EE was walking in fresh snow up a grade to the upper level where his vehicle was parked. He slipped in the snow and fell to both knees, which one leg went behind him. Swelling happened in both knees.

While hauling dirt to dump in a Cat 777D off highway truck, he ran over a rock on the road and bounced in his seat. he hit his elbow on the arm rest, causing a contusion to the elbow.

HE WAS USING A CAT 980B LOADER, WHILE USING IT'S HYDRAULICS AND A CHAIN TO RESET A WATER PUMP. AFTER RESETTING THE PUMP, HE WENT TO DISCONNECT THE CHAIN WHILE STANDING ON THE TIRE OF THE PUMP. HIS FOOT SLIPPED OFF OF THE TIRE ONTO THE GROUND, FRACTURING HIS RIGHT HEEL.

REMOVING DEFECTIVE HOSE FROM RUBBER TIRES LOADER. WHEN HOSE CLAMP WAS REMOVED, THE HOSE END DROPPED AND HIT PELVIS AREA CAUSING HERNIA.

HE WAS CLIMBING DOWNT HE LADDER OF THE ROCK TRUCK AT THE END OF HIS SHIFT. HIS FOOT CAUGHT ON THE LADDER AND HE FELL BACKWARDS ONTO THE GROUND BREAKING HIS UPPER RIGHT ARM.
