Miner stepped off a 22"" high platform injuring his right knee. He has had pain and swelling but did not seek medical attention until 09/13/2011.

The miner was using a utility knife to open a cardboard box. As he was cutting the box, he cut toward his arm; the knife slipped cutting his left wrist. The laceration was given first aid at Carbon Sales, Inc. and he was transported to the emergency room where he received 5 stitches and a tetanus shot.

Employee, who was not on his regular work shift, dropped off a daily newspaper. Upon leaving he stepped off a walkway and sprained his left ankle.

Struck by falling object

Individual picked up a screen to show UPS personnel what was in a damaged packaged received. Screen slipped from hand and fell on right foot. Individual did not realize that his foot was cut until he was home that evening and noticed that his boot was cut.

Fall from machine

EMPLOYEE, AN OVER THE ROAD TRUCK DRIVER, WHOM WAS NEITHER EMPLOYED NOR CONTRACTED BY CARBON SALES INC, WAS STEPPING ONTO HIS TRAILER FROM THE TRACTOR AND SLIPPED FALLING APPROX 5' TO THE GROUN D BREAKING HIS RIGHT COLLARBONE. CARBON SALES EMPLOYEES CALLED FOR AN AMBULANCE AND THE INDIVIDUAL WAS TRANSPORTED TO GEISINGER HOSPITAL, WILKES-BARRE.

EE WAS STACKING BAGS OF COAL ON A PALLET. AFTER BENDING TO PLACE A BAG ON THE PALLET HE TRIED TOSTAND UP AND FELT A SHARP PAIN ON THE RIGHT SIDEBELOW HIS BELT.
