THE INJURED WAS SHOVELING DIRT FROM THE HOPPER OF A AMERICAN SWEEPER WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK AND THE PAIN SHOT UP HIS BACK AND INTO HIS NECK.

THE INJURED WAS CHIPPING AN OLD WELD OFF OF A PIECE OF METAL WHEN A SLIVER OF METAL GOT INTO HIS EYE CAUSING A SCRATCH ON THE EYE. THE INJURED WAS WEARING HIS SAFETY GLASSES AT THE TIME OF THE ACCIDENT.
