Mechanic was looking over the hydraulic equipment on a service truck when something failed & struck the mechanic in head above right eye. Mechanic fell back onto the garage floor.

Employee was attempting to remove a bucket tooth that was stuck in a jaw crusher. Employee hit tooth with a sledge hammer. Tooth shot out of crusher making contact with employee and causing a left leg fracture.

Employee was getting on the dozer at the beginning of the shift. Their foot slipped. They smacked their left knee off the blade.

Employee was completing pre-work inspection on equipment when employee slipped into a piece of metal striking the side of right foot.

Fall from ladders

EE was on a ladder thawing the tail wheel skirting on the spiral plant. When EE was climbing down, the ladder slipped and EE fell approx. 4 feet landing on EE's right side. EE did not seek medical attention until 03/18/19.

employee on street sweeper was on side of road facing east on SR4030 (Gilberton Road). Blue Mercedes behind was almost stopped. Tan buick approached & hit into blue mercedes and pushed it into street sweeper. Local ambulance, paramedic's & police were on site.

Employee lost control of truck while descending a ramp and rolled over.

Employee heard a ""whining"" noise from D65PX, thought a hose may have blown or had a leak. Employee got out of cab & was standing on R side track to look at hydraulic sight glass. Employee slipped off the track and landed on feet on the ground (approx. 3 ft). Employee twisted a knee & jarred their neck.
