The employee was loading a scrap metal plate into a loader bucket. While the plate was being loaded, it slid out of the bucket and cut the lower left leg.

Caught in, under or between a moving and a stationary object

When using a chain hoist to rig a conveyor belt, employee got hand caught between chain hoist and motor. Resulting in a finger laceration to the left pinky.

Trespassing vehicle traveling at an apparent high rate of speed ran into the barricades at pole line 4160 volt. Damage to vehicle and 3 locations along pole line. (Pole, 2 cross arms, and cross arm).

Employee was opening a door on the fines conveyor to unplug the chute. When he took the bolts out of the door the material pushed the door open and pined him between the door and the handrail.

Employee slipped while working on grating. All surfaces were clean and dry. Plant was idle and repair work was being done.

Working on the heavy media prewet screen. 6x6 beams were cracked in half. Screen sees a lot of water, so it was rusty. Took the screen off deck of prewet to be grinded. Was wearing safey glasses and hard had with light attached and also had another light there. Had dust rag, steel could have been on rag when he wiped face. Did not get medical treatment until 8/17/09.

Struck by falling object

EE was removing the guard to get across to clean the chains & sprockets. While lowering guard to ground, it slipped out of his hands and landed on his right foot. Co-worker was also helping him remove the guard. This work was being performed because we were on a mini outage. Employee did not get the treatment until the 9th.

While getting out of the loader injury slipped on the ice and pulled something in his knee.
