The employee was loading a scrap metal plate into a loader bucket. While the plate was being loaded, it slid out of the bucket and cut the lower left leg.

When using a chain hoist to rig a conveyor belt, employee got hand caught between chain hoist and motor. Resulting in a finger laceration to the left pinky.

While climbing a ladder the employee's foot slipped and employee twisted their back.

Prying rock away from door employee hit right ring (4th) finger off C-channel.

While cleaning up scrap steel employee felt sharp pain above belly button. (Date of surgery 11/8/17 - first missed work day)

Employee walked up stairs and had a back spasm.

Trespassing vehicle traveling at an apparent high rate of speed ran into the barricades at pole line 4160 volt. Damage to vehicle and 3 locations along pole line. (Pole, 2 cross arms, and cross arm).

Employee was opening a door on the fines conveyor to unplug the chute. When he took the bolts out of the door the material pushed the door open and pined him between the door and the handrail.

Removing belt & keeper with air gun

EE was washing undercarriage of 2006 Ford F250 pick up truck when cable slipped off the JCB forklift.

Employee was welding pin back in place on left side of bucket and a hot spark went into left ear.

Poor winter conditions, snow covered ice, sub-freezing temperatures. Employee slipped and fell on left shoulder.

While ee was loosening nut on feeder gear box, nut free wheeled cutting finger on bolt that was sticking out.

Employee slipped while working on grating. All surfaces were clean and dry. Plant was idle and repair work was being done.

Working on the heavy media prewet screen. 6x6 beams were cracked in half. Screen sees a lot of water, so it was rusty. Took the screen off deck of prewet to be grinded. Was wearing safey glasses and hard had with light attached and also had another light there. Had dust rag, steel could have been on rag when he wiped face. Did not get medical treatment until 8/17/09.

EE was removing the guard to get across to clean the chains & sprockets. While lowering guard to ground, it slipped out of his hands and landed on his right foot. Co-worker was also helping him remove the guard. This work was being performed because we were on a mini outage. Employee did not get the treatment until the 9th.

While getting out of the loader injury slipped on the ice and pulled something in his knee.

While changing acetylene tank, tank slipped off the frame and pinched finger between tank & frame.

EE was welding inside inlet chute to heavy metals drum and got a foreign body in his right eye.

Lifting oxygen bottles & hurt back. *Employee hurt himself on 9/15/08 & did not tell us until 9/18/08.

Shoveling material into wheelbarrow and hurt his back.

While operating a 5 ton chainfall, employee felt a nip in his stomach.

Employee was prying on gear box with pry bar and hurt hard when gear box left loose he jarred shoulder and hand.

EE leaned up against previously cut hot plate and burned his right forearm. EE also has poison ivy on both arms, not sure where on the job he got that.

While cleaning mud accumulation out of discharge chute the air lance got stuck in the muddy material causing employee a strain of the left shoulder.

While lifting iron plates and screens ee hurt his back.

WHILE LIFTING PLATES EE HURT HIS SHOULDER AND NECK.

WHILE USING AN AIR CHISEL, A FROZEN LUMP OF MATERIAL BROKE FREE WITH THE TOP OF THE RIGHT HAND GETTING PINCHED BETWEEN THE HANDLE OF THE AIR CHISEL AND IRON FRAME OF CONVEYER.

WHILE HOOKING UP TRACTOR TO TRAILER, HE TWISTED LEFT KNEE.

WHILE USING A HAMMER AND CHISEL EE STRUCK HIS LEFT POINTER FINGER.

WHILE SHOVELING OUT SCRAPER BUILDING SMASHED HIS RING FINGER ON HIS LEFT HAND.

WHILE SWINGING A PICK HANDLE EE HAND CAME IN CONTACT WITH THE CONVEYOR LINE. HIS LEFT MIDDLE FINGER GOT CAUGHT BETWEEN THE PICK HANDLE & THE CONVEYOR LINE. HIS FINGER WAS BUSTED OPEN
