Employee was standing on the ground and when EE stepped up to get into the forklift and bumped nose against the outside of the ROPs that resulted in a cut across the bridge of the nose. EE went to the hospital and received 4 stitches, and was released to return to work the next day with no restrictions.

On Wednesday August 24, 2022 the operator of a 2DP600 forklift was returning unloaded from the truck loading area to the packaging area. While transitioning between 2 warehouse bays the driver was distracted by nearby activity and drifted into a support column. A piece of broken mirror came in contact with the driver's forehead causing a laceration that required 7 stitches.
A Securitas guard lost balance while exiting an office, grabbed / slid down a door jamb catching the latch plate cutting the forearm. Securitas supervisor was called, arrived, could not take for treatment (Sup. was COVID positive). Ambulance called, treated received stitches to close the 8" wound, and admitted for observation and released Monday 7/25/22.

While the employee was tramming an empty forklift he came in contact with the base of a I-beam support. The pier of the I-beam was constructed of several layers of mortared brick. The mast of the lift dislodged the upper layers of brick allowing the front left wheel to ride up onto the remaining brick support causing the forklift to tip over on its side.
