Contact with electrical current

EE was installing a new disconnect box in the Sky Brothers MCCH3 trailer and was using a hydraulic hole punch in an energized panel and the hydraulic hole punch fell and contacted the live voltage that was not locked out and/or de-energized. EE received 1st, 2nd and 3rd degree burns to 14% of their body, mostly on the right side, also hands and left thigh.

Caught in, under or between a moving and a stationary object

Employee was removing link arm assembly from loader, he reached to pull the pin holding the arm, when he pulled the pin it moved pinching his hand between the pin and boom of loader. The employees hand was cut requiring medical attention/stitches

Struck by falling object

Employee was changing wear plates inside of a crusher. He was striking the plate with a hammer to loosen it. When the plate became loose, it slid down onto the employee's fingers.

Fall from machine

Employee with 30 years of mining experience fell from a conveyor structure and landed on his side. He never lost consciousness and complained of pelvic pain and tight breathing. He did not the JLG man-lift with harness to access the elevated surface as trained.
