Struck against a moving object
bulldozer drove over a bump and operator bumped head on inside of cab. Operator received stitches to forehead and returned to work.

Caught in, under or between a moving and a stationary object
Miner was helping install a crusher wheel with an overhead trolley system. When the trolley got jammed the miner tried to free it, his left little finger got caught between the trolley wheel and the I beam causing it to be cut.
