When stacking stand up broken stone, EE was putting smaller rubble into skid steer bucket stumbled over some stones and hit his left knee on skid steer bucket. The bucket was not attached to a skid steer at the time of injury. Went to E.R. had x-ray returned to have MRI no restrictions at this time.
Stacking stand up broken flag stone when a piece of stone came back off the pallet sideways pushing against side of right knee.
Stacking stand up broken flag stone onto a pallet when walking a stone to the pallet he twisted his body and felt a pull/pain in his lower back.

Wet weather, mud on the boots of skidsteer operator. When he exited the skid steer, (New Holland 190) he slipped off the forks and was falling. He caught himself before he hit the ground twisting his right hip and back.
Lifting large stone from ground, felt a ""pop"" in right shoulder with alot of pain. also injured right hand when stone fell and crushed hand between stones.

When lifting a piece of 1 1/2"" thick stone from the ground to stand on it's side, a ""sharp"" pain was felt in his middle lower back with tingling down left leg. Pain/ache continued and worsened.

Employee was stacking stand up broken stone, when he was smashed his pinkie finger between stones, at 255 he complained it was swollen and he thought it was broken.
Complained of lower back pain on 7/16/08 when stacking standup broken stone. Went home at 1 pm. He did not go to hospital (but was encouraged to). Returned to work 7/17/08 & stated he was OK. Went to hospital 7/23/08 with complaint of lower back and leg pain.
EE c/o having pain in his back when bending and lifting stone. He was stacking garden steppers & colonial at the time with Other EE, and Another ee. he then finished wraping his colonial perfect after that other ee took him home.
Right wrist pain when picking up stone. Stated he was unable to hold the stone and needed to go to the hospital to be seen.
EE was in the area where he had been stacking stone. He removed his safety glasses to wipe sweat off of his face, in doing so, he got a small piece of stone up under his L eye lid. Another EE was working in the same area, he witnessed what happened.

EE was attempting to stack a piece of stone on a stone pallet. The corner of the stone broke off causing the rock to fall on him. Co-worker lifted the stone off his body.
Employee was putting a piece of stone into a standup pallet from the top. The stone slipped causing him to ""pinch"" his left pinkie between two stones. As a result he has a deep cut on the pad of his left pinkie.
When assisting co-worker to put 2nd piece of stone on a standup broken pallet, the first stone fell over sideways and struck employee in the right side of his head, then hit his left arm. Small laceration on right side of head and bruises on left arm and shoulder.
Employee was putting a piece of stone on a standup pallet. A piece of the stone broke off and lacerated his right lower forearm.

EE was doing standup, he slipped on stone while walking stone onto pallet. When he slipped, the stone came down on his back, it broke and a piece hit him on right lower leg.
EE was doing 2-2 1/2"" broken standup when his foot slipped, to not drop the stone onto himself, he swiveled, then had bad pain down his legs and in his lower back. Numbness in both legs.
Left thumb was injured when filling in a standup pallet. Left thumb was smashed between two stones.
Lifting stone onto pallet for another employee. Lifting stone on standup pallet severe lower back pain and down left leg pain.
Pulling standup, pallet slipped out of hand, came down on back of left leg causing a bruise which became black and blue.
Cut, puncture to right elbow. Fell hit elbow on stone, causing puncture and cut to right elbow.

Employee was stacking a standup (broken) pallet 2 1/2- 3"" when a piece of stone broke off and punctured his right forearm causing a puncture wound which required 4 stitches.
