Duffy Inc · Fall to the walkway or working surface

Was going to shut machines off slipped and fell shoving heal of hand into ground. EE cont'd. to work then noticed hand was starting to swell and the ring and pinky fingers were numb. The Dr. stated to be off rest of the day and to wear a splint for 7 days.

Stepping off crusher and slipped. Twisted ankle returned home felt ok over weekend never mentioned incident. Returned to work 12/21/09 stepped off excavator track and hurt same ankle, went to Dr. on 12/22/09 sprained, doctor recommended 1 day rest and or brace.

Piece of iron stock fell from lid of crusher while doing maintence and hit EE on L elbow. The EE was attempting to beat a bolt loose with a piece of steel, EE said he was fine , but foreman offered to take EE to ER, but he refused. Foremen insisted and EE get it checked out by DR. Foreman called everyday till EE went to see DR.
