20' x 10' x 2' piece of roof fell. Piece of rock fell on boom of scaler and came thru windshield striking operator in the ribs

Employee was dismounting John Deere 944 Loader at end of shift and slipped off the ladder injuring left ankle when EE hit the ground

Driver was driving CAT 773E loaded headed up the haul road from the mine portal when EE heard an explosion and the Truck burst into flame. Driver climbed out of cab and jumped from truck ladder approximately 3 1/2 feet to the ground injuring EE's right Leg. Fire dept was called .

Employee was cutting hose with pocket knife and knife slipped puncturing left forearm. Employee received 5 stitches.

Fall to the walkway or working surface

Employee was working on Sand Plant electric line project. The early morning snow had been plowed which created icy conditions. The employee walked around the corner of a 305.5 excavator when EE lost footing. When EE fell EE landed on the left knee taking all of the weight.

Over-exertion in lifting objects

EE was picking up metal plate from the ground to throw in the back of a pick-up. EE estimated metal plate weighed about 150-200lbs. EE rested a bit before calling EE's father to take EE to the ER. The ER stated right erector muscle strain and to take ibuprofen follow up with Occupational Health on 4/5/2016. Cleared to return to work on 4/6/2016 with no restrictions.

ON 11/24/04 EE WHILE LIFTING STEEL PULLED MUSCLE IN ABDOMEN & DIDN'T REPORT IT. HE THOUGHT IT WOULD HEAL. REINJURED IT ON 12/27/04. MORE PAIN BUT THOUGHT IT WOULD HEAL. REPORTED IT TO OFFICE ON 3/24/05. PAIN WAS SEVERE AND NEEDED MEDICAL ATTENTION.
