EE was hauling shot rock when another machine operator approched EE went to his right and hit a frozen pile of rock which was covered with snow causing the truck to shift to the left. This caused the EE to hit his head and leg.

Drill operator was drilling blast holes. After breaking a drill steel, one had to be put back on. While adding the drill steel onto mast, the drill hammer was turning. When the striker bar of the hammer threaded into the drill steel it began to rotate wrapping the operators glove, sweat shirt & eventually his arm around the drill steel causing amputation of right arm above elbow.
