Employee fell approximately six feet from an extension ladder while putting supports under a belt line at the crusher and injured right foot/heel.

Employee was mounting cheek plates on stone crushing equipment. While tightening bolts on the plates ee hit left hand with the hammer wrench. Employee went to emergency room and treated for a lacerated/fracture middle finger. Finger received 1 stitch and a bandage and splint applied.

Commonwealth Quarry Inc · Struck by falling object

EE moving steel plate on 2 metal saw horses with wheels. Sliding on concrete floor of shop. Wheel caught and horse flipped, plate fell on feet. Coworker lifted plate off. Sitting in chair & said "he felt like he was going to pass out". Passed out for 20 sec. Transported to hospital by ambulance. EE wearing steel toe boots. Treated in the emergency room for a broken toe, left foot.
