Lowering a jaw plate into jaw, hook brakes on rental excavator. Miner tripped, hard hat fell off, shakel came loose and hit miner on top of the head.

EE WAS WELDING ON LOADER BUCKET AND WAS BENDING A PIPE AS HE WAS WELDING OR HEATING IT UP WHEN PIPE SNAPPED AND PIECE OF METAL HIT HIM ON LEFT SIDE OF FACE. PIPE ALSO HIT HIM ON HIS CHEST.
