Material Sand & Stone Corp · Struck against stationary object

We were building a new ramp at the quarry and as we were building the berms the operator got too close to the edge as EE was attempting to dump and the truck went over the side. The equipment involved was a haul truck, manufactured by Caterpillar, Model 769D.

While changing bucket on an excavator, EE was pulling pins and pin slipped and EE tried to catch it and got a finger caught between pin and the bucket.
