Employee twisted left ankle. Slight sprain.

Moving equipment - back muscle issue.

Employee lacerated finger while removing a stone wedged in palletizer. Received stitches in right thumb. Will return to work for light duty after 4th Holiday.

Impacted right thumb when moving a screen. Was reviewed at local urgent care. Ok. No time lost.

Pulled muscle in shoulder while performing plant maintenance. After medical review, suggested that EE relax for a day or two.

Worker cleaning tools created a flash resulting in facial burns. Was treated and released by medical facility.

Removing stone from troughing roller without shutting down. Hyper extended a few fingers.

Wrist sprain

Standard GM activities. Lifting material.

Muscle pull on right side. Medical review indicated OK to return to work. Discomfort persisted. Continuing time off with muscle therapy.

Pulled muscle in shoulder.

Pulled back muscle, during twisting motion, while using a grinder.

Employee was lifting a grate and hit left knee, resulting in a contusion.

Bruised hand tensioning bolt. The worker did not mention the issue until the next morning. Office dispatched him to medical. Released with hand bruise (splint/ice) to modified duty pending specialist review. Office recommended off duty pending specialist review next week.

BRUISED RIGHT HAND WHILE REPLACING SCRAPER RUBBER. ALLOWED TO RETURN TO LIGHT DUTY.

Ee was changing screens on CEC platform which is approx twelve feet above ground level. He misstepped through ladder opening and fell to ground level. He was dispatched to hospital.

SEALER UNIT FAILED, POWER SHUT OFF AND CHAIN GUARD REMOVED. OPERATOR ENABLED POWER TO REMOVE CHAIN AND CAUGHT FINGER IN CHAIN. TIP OF LEFT INDEX FINGER SEVERED.

Hit left hand with grinder, cleaning up shaft.

MOVING A BLOCK OF WOOD UNDER A LOADER AND HURT LEFT SHOULDER BLADE & FRONT CHEST - PULLED MUSCLE??

LOADING BATTERY CHARGER IN BACK OF PICK-UP TRUCK AND BACK WENT OUT.

WHILE GRINDING SOMETHING FLEW IN HIS RIGHT EYE.

WENT TO PICK UP EMPTY BAG AND SLID ON ICE - FRACTURED RIGHT ANKLE.
