Crusher stopped working. Employee ran to stop plant. Ran and ducked under conveyor, lost balance, hard hat was knocked off and he cut head on inside of frame of conveyor belt. Stitches received.

Employee was shoveling material from catwalk on the screen plant resulting in strain to triceps and deltoid, employee is on restrictions.

Employee was removing rock from tire, bubble formed on tire and tire exploded causing fracture to left hand.
EE was using water pump hose and it twisted so water was not reaching the tank. EE released the camlocks to adjust hose and scalding hot water escaped from the hose and gave him 2nd degree burns on his inner thigh.
Right thumb got caught in the loader door.

He took gas can to fill pump; set gas can down and foot slipped. He caught himself by reaching out and left hand came in contact with hot muffler shield, resulting in an immediate burn on the left hand.
