The injured was replacing a lid or cover back on the roof bolting machine. He strained or pulled something in his lower back.

Went to plug in shuttle car cat head up to power center, something shorted out and caused a flash and burned EE's right hand.

Victim was installing roof bolt, he reached to drop head but activated boom swing. causing boom to strike his head pushing him back into coal rib.

He twisted left knee while making belt set up. The first day absent from work was 2/15/2008.

Riding mantrip, his hard hat got caught on water lines pulling head & shoulders into roof.

Loading a box of resin on the roof bolter, pulled something in right shoulder.

Employee was shoveling belt line while on his knees twisting his right leg at the knee.

Roof fall, laminated shell and sand.

Shuttle car operator got his head between roof of the mine and car, causing light concusion and tissue tear.

Victim was crawling and placed left knee on rock. When he put weight on knee, the knee twisted.

Employee was using a hand drill to drill a hole in a piece of metal. The drill bit hung causing the drill to twist employee's right hand.

Employee raised up to get a piece of candy from the top of the MBC. His right foot was outside the operating compartment and was caught between the MBC and the belt structure.

Employee was putting coveralls on standing next to the shuttle car when a piece of wood or crib block fell striking big toe on left foot.

Employee was greasing loader. Wind blew dust in his left eye.

Employee was lifting rub rail back on miner.

Employee was straighting a roof bolt and pulled something in his right arm.

A piece of draw rock fell & cut EE's right arm.

Employee said he squatted down and his knee popped.

Employee was drilling a hole. The steel stopped up causing dust to blow back in employee's face.

No. 6 Entry between break 30 & 32, 2 breaks inby spad 834. The roof strata was soft shale with a fault. The fall was 75 ft long 20 ft. wide 6.5 ft high.

Employee was putting a plate on a roof bolt. He bumped his forearm on the head of the roof bolt cutting his forearm.

Rock fell & hit EE's right knee causing pain.

Employee was unloading belt structure from a utility trailer. As he was pitching a piece of structure, it hung his light cord causing him to mash his right thumb between the structure and trailer.

Roof bolting, a piece of draw rock broke and hit left ring finger.

A piece of draw rock fell out between roof bolts striking employee between the neck & shoulder resulting in bruises only.

The face boss on the section was crawling between lo lo structure and coal rib.The front of the MBC hit tailpiece and shoved the lo lo belt structure mashing the victim's legs against the coal rib.

Loading parts up at the office to bring to the mine, employee hurt his lower back.

Removing the boom from a roof bolting machine, the boom came off the machine unexpectedly and pulled the employee's left arm & shoulder.

A piece of rock 6' X 3' X 4"" fell from the rib to the bolt, striking employee in the head & shoulders forcing him to the floor. The rock also hit the roof bolting machine on the way down.

Accelerator pedal didn't return to neutral position causing operator to hit rib.

A piece of rock fell from the rib striking right foot of operator.

While operating roof bolter, a piece of rock fell between last row of bolts and TRS striking EE on neck and left shoulder.

After picking up a pry bar, EE reported severe pain in right lower back.

While bending roof bolt, EE slipped and fell, striking his chin on a roof bolt plate, resulting in a cut on chin.

Right knee swelling while crawling in mine, possibly from small rock lodged between knee pad and knee.

While operating roof bolter, EE caught left thumb on boom arm, resulting in mashing left thumb tip.

While employee was cleaning water sprays on miner, a piece of draw rock fell from roof striking his upper back.

Shuttle car tire flipped a rock from mine floor, striking employee's chin.

Employee was riding mantrip out of mine when a piece of debris fell from mine roof and went into employee's ear.

Employee picked up a box of roof bolt glue and turned with it, straining lower back.

WHILE OPERATING A ROOF BOLTER A PIECE OF ROCK FROM THE MINE ROOF FELL AND STRUCK EMPLOYEE'S RIGHT LEG.

EMPLOYEE WAS REMOVING A ROCK FROM OFF CONTINUOUS MINER, ROCK SLID STRIKING LEFT THIGH AND KNEE.

EXTENSIVE CRAWLING IN MINE HAS CAUSED EMPLOYEE'S LEFT KNEE TO SWELL.

Employee was lifting conveyer belt to install rollers and felt pain in lower back. Possible back strain.

While crawling in mine, employee twisted right knee. causing swelling.

While operating roof bolter, employee caught left thumb on bolter head. Crush injury to thumb.
