Caught in, under or between collapsing material or buildings

Donning fall protection equipment, the miner entered the top of a primary feed hopper without the knowledge of management and contrary to mine safety procedures to remove a large rock. While trying to break up the rock, raw material that remained on the sides of the hopper sloughed off engulfing the miner.

Employee was installing a bucket cylinder pin with pry bar, the end of the pinch bar broke off and jammed their hand against the corner of the auxiliary valve

EE was helping put a return roller on a primary conveyor at the secondary plant. A second miner was holding the conveyor belt up with a pry bar when the pry bar slipped and struck EE on the head while wearing a hard hat.

Struck by falling object

While working under an excavator, dirt fell on EE's head and rolled behind the safety glasses. After cleaning and rinsing eyes, EE resumed work. A day later, EE's right eye still felt irritated.

Over-exertion in pulling or pushing objects

EE was washing the tunnel and part of the hose was covered with material, so EE yanked on the hose to get it loose and felt pain on the inside part of EE's right wrist.

EE was operating the excavator when EE started feeling increasing pain in the lower back. When EE started to exit the machine, EE felt a sharp pain in their lower back that shot down EE's left leg and EE's toes started to tingle. EE crawled/slid down the machine and could not stand up from the pain.

Shortly after welding a steel plate, the plate broke off and struck the left side of the miner's face.

While walking down a downward slope to a hopper, EE fell backward and landed on their right side.

Employee was using a pry bar to force a rollaway door to close. Employee was using extra pushing force with right arm and felt a pulling sensation at the base of the back of neck.

While attending a monthly safety meeting in the breakroom, employee felt overheated, stood up, and briefly fainted.

Employee was descending CAT 374 Excavator and when their left foot stepped on the ground, their left knee hyperextended.

Fall onto or against objects

Employee was climbing on a CAT 980K Loader and missed the top step. As EE slipped, EE went to catch self and hit right hand on the cab and heard a pop.

While shoveling, EE twisted and felt pain in right side of lower back.

Struck by falling object

EE had lowered a ring gear with the truck crane and was using hands to guide the ring gear off the shaft when it fell 3/4"" and smashed EE's fingers causing multiple lacerations on both hands.

Fall from machine

While servicing excavator 374FL, EE started to climb down steps when EE's hand slipped causing EE to fall down and get a laceration on left arm. After sitting down safely, leaned forward and fell causing scrapes on the right side of face.

Water Truck right tire pressed against sweeper causing it to deflate, and shifting the weight and making machine slide onto outer wall of the MCC room, causing metal box on interior wall to strike EE on right shoulder. Injury was considered first aid only until 3/9/22 when specialist prescribed physical therapy.

Employee opened metal door to the second discharge chute on the dry plant to remove a piece of wood that got stuck when the wind caught the metal door slamming it against middle right hand finger, causing pain and bleeding.

Employee was removing rocks that were wedged in the Feed Belt discharge chute at the Dry Plant. The chute has a metal door that was required to be opened to get to the wedged rocks. A gust of wind blew the door closed and slammed on the employee's left hand middle finger, causing a deep laceration.

Employee was changing wear parts on impact crusher when EE felt hard pain in the abdominal area.

Struck by flying object

EE was performing maintenance on the ripper tooth of the dozer. When EE hit the tooth with a hammer to loosen it, a small piece of metal chipped off, went through EE's pant leg and embedded in the right shin.

On July 2, 2019, employee rolled left ankle while getting on a step and then on July 3, 2019, twisted left ankle again when inadvertently stepping in a hole that was not visible due to standing water at the primary plant under the hopper.

Employee was driving loader too fast on bumpy road and the jolting motion caused a strain in EE's lower back

EE was driving to the pit to load when EE saw another truck coming out of the pit. EE attempted to slow down and give the other truck the right of way, but in the process, jack knifed the truck. The steering wheel bruised EE's left leg.

EE opened left side access door on Bulldozer to change oil filters. While doing task the EE squatted on the top of the tracks and the door closed and struck the EE's right knee. EE suffered a bruise to his right knee.
