EE WAS NOT SURE WHEN, HOW, OR WHERE INJURY OCCURRED, LEFT EYE WAS INFECTED AND HURTING WHEN EE REPORTED TO EMPLOYER ON 5-24-02, AND SAID THAT SOMETHING MAY HAVE GOTTEN INTO HIS LEFT EYE WHEN H E WAS RUNNING THE GRINDER ON 5-11-02. EE WAS SENT TO THE EYE DOCTOR AND A SMALL PIECE OF METAL WAS REMOVED. EYE WAS INFECTED AND HE IS STILL SEEING THE DOCTOR.

EE IS A DIABETIC AND HIS BLOOD SUGAR GOT TO LOW WHICH CAUSED HIM TO FUNCTION IMPROPER. WE GAVE HIM SOME CANDY BUT IT DID NOT HELP SO WE CALLED 911 FOR AN AMBULANCE WHICH ARRIVED ABOUT 15 MINUT ES LATER. THE MEDICS TOOK HIS BLOOD SUGAR READING, THEN ADMINISTERD THE PROPER MEDICATION. HE WAS THEN BACK TO NORMAL. HE WAS TAKEN TO THE HOSPITAL FOR OBSERVATION THEN RELEASED ABOUT 8:00 PM

OPERATOR CAME TO WORK WITH A SWOLLEN LOWER EYE- LID, COMPLAINING THAT SOMETHING WAS SCRATCHING HIS EYE. WASHING THE EYE WITH COLCYRIUM EYE WASHDID NOT HELP SO HE WENT TO THE DOCTOR THE NEXT DAY AND THE DOCTOR REMOVED A FOREIGN PARTICLE FROM HIS EYE.

LOADER OPERATOR WAS STEPPING DOWN FROM LOADER AND MISSED THE LAST STEP AND AT THE SAME TIME HE JUMPED FROM THE LOADER AND LANDED ON THE SIDE OF HIS FOOT VCAUSING A TENDON TO BE STRETCHED IN TH E ANKLE.

TRUCK DRIVER WAS HOLDING UP A PIECE OF THREADED ROD THAT WAS SCREWED INTO A SHAFT. THE OTHER END HAD A SLAPHAMMER ATTACHED TO IT. WHILE ANOTHER MAN WAS WORKING THE HAMMER THE THREADS BROKE OFF AND ALLOWED THE ROD TO SLIP AND CUT THE DRIVER'S HAND BETWEEN THE INDEX FINGER AND THUMB. AFTER FIRST AID WAS ADMINISTERED HE WAS TAKEN TO A HOSPITAL EMERGENCY ROOM WHERE HE HAD 4 STITCHES PU
