EMPLOYEE WAS UNHOOKING TWO WHEEL TRAILER FROM PICKUP ON INCLINE FAILED TO BLOCK WHEELS ON TRAILER SECURELY AND TRAILER TONGUE ALLEGEDLY HIT HIM IN STOMACH WE WERE NOTIFIED TODAY 041483 BY OUR INSURANCE CO THAT MR WYCHE HAD SURGERY FOR A HERNIA AND THAT IT WAS JOB RELATED WE ARE SUBMITTING THIS REPORT IMMEDIATELY UPON RECEIVING THIS INFORMATION
