The employee was pull-starting a generator in the back of a pickup-truck. The handle came un-tied, and the employee fell backward out of the truck bed. She suffered a sprained neck and was placed on light duty by a doctor. No permanent damage is suspected.

Contractor was burning debris behind the mine maintenance barn without required notification of approval. According to the owner, he thought the fire was out and left the mine site without the appropropriate fire watch. The fire was extinguished the first time by the fire department at 2123 hours. They were called back again at 0115 am. Approx. 1.8 acres burned.

A FIRE OCCURRED AT IMPOUNDMENT ID NO. 1211-TX-09-02847-17 (SP-4). UPON INVESTIGATION THE FIRE BEGAN FEB. 15, 2002. THE FIRE WAS NOTICED BY A TMPA EMPLOYEE. SOURCE OF IGNITION IS BEING INVESTIG ATED AS OF THIS DATE.

WHILE CUTTING HAY, A NEARBY LAND OWNER STRUCK A ROCK THAT SPLIT, SCATTERED, AND IGNITED. THE FAMILY TRIED TO EXTINGUISH THE FIRE. THE COMPANY WAS NOTIFIED BY THE FAMILY AT 3:15PM TO REPORT THE FIRE. THE COMPANY DISPATCHED THE AGENCY'S EMERGENCY RESPONSE TEAM AND NOTIFIED THE FIRE DEPT. APPROXIMATELY 250 ACRES IN THE MINE AREA BURNED. FIRE WAS EXTINGUISHED AT 5:30PM.

OWNER OF WILLIAMSON TREES WAS PLANTING TREES IN A1-1 SECTION. A PICKUP TRUCK WAS TRAVELING NORTH ON GRIMES COUNTY ROAD 190 PULLING A BARBECUE PIT ON A TRAILER. LESS THAN 30 SECONDS AFTER THE T RUCK PASSED, EE NOTCIED A FIR IN THE SW CORNER. EE COULD NOT EXTINGUISH THE FIRE. HE WAS BY HIMSELF W/NO COMMUNICATION. HE DID NOT KNOW IF THE BARBECUE PIT WAS LIT. THE FIRE WAS EXTINGUISHED
