Mechanic was standing by service truck with back to machine when Bed Rock mechanic moved excavator and pinned 1st mechanic against service truck with the bucket.

Employee had opened a box that contained a pressure washer. The employee then was trying to pull a tag off of the machine (pressure washer). At that moment he pulled out a personal knife to cut the tag off. The knife went through the tag and his left hand (thumb)causing a laceration.

WHILE EMPLOYEE WAS UNLOADING A HYDRAULIC CYLINDER FROM A PICKUP TRUCK, THE CYLINDER FELL WHEN IT SLIPPED THROUGH THE CHAIN & LANDED ON HIS FOOT FRACTURING HIS GREAT TOE.

EE HELPED TO PERFORM MAINTENANCE ON A SCREEN BOX, REPLACING SCREENS. THE NEXT DAY, HE COMPLAINED OF SORENESS TO HIS BACK. DIAGNOSED WITH BACK STRAIN.

EXPERIENCED MINER ACCIDENTALLY STEPPED ON A ROCK, & TWISTED HIS ANKLE. SEEMED TO BE SPRAINED, BUT HOSPITAL TALKED LIKE IT WAS BROKEN.

EXPERIENCED QUARRY FOREMAN GOT HAND CAUGHT IN PULLY. HAND WAS CUT OFF, AS WELL AS A THUMB ON THE OTHER HAND. PULLY HAD BEEN GUARDED BUT GUARD WAS REMOVED DUE TO SERVICING.
