Employee was stacking stone in box off of the chopper & smashed a finger when loading the bucket

Gantry gear smashed 3 toes on workers R foot.

Pinched Finger under rock - R Ring Finger

EE smashed left middle finger with a rock on splitter 4 pushing the stone into the splitter. EE immediately came to the office to bandage the finger. Another ee tended to the wound and we took ee to the Dr to have it checked out. No reported breaks, finger was cleaned and bandaged and sent home to heal.

Employee burnt the top of foot with the pressure washer after company hours.

Employee jammed finger between 2 rocks when laying the stone onto the pallet from the splitter.

EE said that EE was pulling a break drum off parked near the shop. EE slipped and fell backwards onto EE's left hand. The injury was a broken bone to EE's left wrist which was casted. EE was off work for 2 weeks and on restricted duty for 6 weeks. The injury occurred after working hours.

EE was using a tool to leverage to install a nre belt on the water saw, when the belt went into it's proper place his finger was in the way, EE's pinky finger was cut along the side and undernail.

Removing belts and installing new belts on the water saw his finger became trapped between the belt and pulley cutting his left index finger.

Pushing rock through hydro splitter, it got stuck but didn't inform other workers, the other worker pushed the button to chip the rock but EE's finger were in there, too.

EE was chopping rock and his finger was trapped between two rocks causing the left index finger nail to be ripped up.

Employees were working in the shop. It was raining so they closed the shop garage doors. EE was closing his door and injured employee didn't notice. The door hit him on the bridge of his nose.

WORKING ON BOBCAT TRACTOR AND WAS PINNED BY BUCKET.

WORKING ON STONE SPLITTER AND CAUGHT THUMB IN IT.

EE WAS MOVING A ROCK AND IT SLIPPED, CRUSHING HIS LEFT INDEX FINGER.

ON STONE SPLITTER AT QUARRY 3.7 MILES WEST OF I-35 WAS CUTTING STONE WHILE MOVING A STONE ANOTHER STONE FLIPPED & MASHED HIS MIDDLE FINGER ON RIGHT HAND.

PUSHING STONE THROUGH CHOPPER & SMASHED RIGHT HAND INDEX & MIDDLE FINGER.
