Struck against a moving object

Front End Loader was struck by a Haul Truck causing to flip on its side. The front end loader operator received right shoulder and neck injuries.

Employee was trying to prevent equipment door from closing, so he put his hand in between, and got his right hand smashed with the door, and his middle finger got broken.

Accident type, without injuries

End dump trailer tipped while dumping into crusher bin. Inspection showed 8' tilt in surface area and 25+ mph wind contributed to tipping of trailer. No injuries. Had area leveled.

EMP AND OTHER WERE CUTTING STEEL BAR WITH OXYACETYLENE RIG WHEN HE CUT BAR IT FELL STRIKING LEFT THUMB, BROKE DISTAL PHALANGE.

NOTE: WAS FIRST AID THEN BECAME REPORTABLE ON 03/02/04. EMP WAS AIRING TIRE & WHEN IT POPPED/SEALED IT BLEW SOME DUST IN HIS LEFT EYE. EMP INJ BEING TREATED FOR INFECTION.

WAS MOVING MATERIAL, HE DROVE TO TOP OF A PILE, STARTED COMING DOWN, BACK TIRES CAME OFF GROUND & LANDED AGAIN JOLTING HIM IN THE SEAT.

EE WAS WORKING ON A SAND SCREW. HE BENT OVER TO PICK UP A WRENCH AND HIT HIS HEAD ON THE EDGE OF THE SCREW FRAME.

EMPLOYEE WAS CHANGING A HYDRAULIC HOSE WHEN HE LOOSENED THE FITTING SOME OILY DIRT GOT IN HIS EYE. EYE WAS IRRITATED SO WE SENT HIM TO THE DOCTOR TO EXAMINE IT.

CHECK VALVE IN SUCTION LINE WAS STUCK OPEN. EE DUNKED HIMSELF UNDERWATER TO WORK VALVE INSTEAD OF REMOVING SUCTION HOSE.
