Loader operator was jolted in loader and suffered head injury.

Fall onto or against objects

EE was standing on the back of pickup truck and disposing of scrap metal. The weight of a bucket load of scrap metal pulled EE forward causing EE to fall and striking the right side of EE's face on a metal object, and also injuring EE's right shoulder.

Employee was lifting a bucket pin with co-worker on a Cat 374 Excavator, and immediately after started feeling tightness and pain in the lower back.

Employee was climbing down from a loader when EE lost grip and fell to the ground, landing on EE's right side.

Co-worker and IE were removing a bolt and nut to remove a troghlin idler. IE was holding a combination wrench to hold the nut from moving and the combination wrench slipped and pinched IE's right index finger against the angle iron and wrench.

Changing cutting edges on Caterpillar 972 front end loader. While holding bolt head, the bolt slipped causing him to cut his left hand.

Employee was getting out of a Euclid R25 that was parked next to walkway/platform, lost his footing causing him to slip and fall. When he landed he felt pain in his left ankle.

Injured was walking down smooth gradual slope & twisted his left ankle. Initially he thought it was a mild sprain, however, within a couple of days it became inflamed & he was taken to the doctor. X-rays indicated a small fracture in his ankle. Because the injured is diabetic & less likely to heal naturally, the orthopedic dr. recommends surgery to minimize complications.

EE was realigning elev chain on 623G by placing log between chain & sprkt while engaed to make chain skip tooth. The log rolled across outer housing pulling his hand across surface smashing/severing tip of right ring finger. He rtw w/in 2 days w/restrict due to pain meds. He chose not to take meds and was allowed to operate 623G as long as no meds were taken. EE retrained.

Small metal fragment broke off of excavator tip and lodged in injured's bicep. ER Dr chose not to remove item b/c no danger as did surgeon. Injured to return in one week for removal of 2 stitches. No lost time/No impair
