EMPLOYEE WAS ASSISTING IN REPLACING BELTS ON A CRUSHER. MECHANIC INSTRUCTED WORKERS TO PULL HANDS BACK BEFORE ROLLING BELTS ON. EMPLOYEE KEPT HIS HAND NEAR THE BELT, AND IT WAS PINCHED BETWEEN THE BELT AND PULLEY. THUMB WAS CRUSHED AND APPRROX 1/4 INCH HAD TO AMPUTATED.

EE WAS WORKING ON THE GROUND RIGHT BESIDE JAW CRUSHER WHEN A ROCK GOING INTO THE CRUSHER BOUNCED OFF AND LANDED ON EE'S BACK. RESULTED IN A BRUISED AND SORE SHOULDER. EE WAS CHECKED BY A DOCTO R BUT MISSED NO WORK.

EE WAS ATTEMPTING TO REMOVE A ROCK FROM A TAIL PULLEY. RATHER THAN WAIT FOR THE PROPER TOOLS TO REMOVE THE ROCK HE ATTEMPTED TO REMOVE THE ROCK WITH A HAMMER. WHEN THE ROCK DISLODGED, THE PULL EY TURNED, CATCHING THE EE'S ARM IN IT AND BREAKING IT.
