When getting down from trackhoe after filling up the greaser, he slipped on some grease and landed on his right shoulder. Fall resulted in dislocation of right shoulder.

Operator of skid steer loader had his foot sticking outside the cage and brought boom down onto his foot. Pinched his foot (no broken bones). He has been told to not work for 7 days.

While replacing a blowbar on crusher, they were using a hammer to knock blowbar loose. A metal shard from blowbar hit employee in the leg and made penetrating wound. Metal was removed and wound sutures were required

While climbing into Volvo 360 trackhoe, he indicates his foot slipped out from under him and pulled a muscle in his back. He did not fall or hit anything. Did not report the event until 6:30 p.m. that night and refused any medical treatment. On Monday (3-23-09) said back still hurting and wanted to go Dr. Dr. said it was strained muscle.

Contact with electrical current

EE gave conflicting story; however, I believe he tripped breaker with door open and was shocked. No evidence of electrical burns or damage. All tests at hospital showed not damage.

Employee was removing blow bar from crusher unit using a small hammer to dislodge blowbar. A small piece of the blow broke loose and lodged in his arm. We took the employee to clinic to remove fragment. Injury will required a few stitches. No loss of function of arm.
