Employee was taking off the wheel chock by sliding it off the holder; the weight of the falling chock hurt his shoulder.

Employee was walking to a tree to cut it down when suddenly he felt a pop in his knee and fell to the ground.

Employee was adjusting V belts in the primary crusher. As he was holding the belt with his right hand and pulling the belt with his left hand, his leather glove was caught between the sheave. He injured his left hand ring finger.

Employee was removing V belts on the primary crusher to replace with new ones. He was working on cutting the final belt on the motor. As he was holding the belt with his left hand and cutting with his right, his hand shifted, bringing it into the path of the razor knife. Employee cut through his leather glove and lacerated his left hand.

EE was doing normal job using a welding machine and grinder. It was windy and debris flew into left eye

EE WAS BACKING HAUL UNIT UNDER LOADER TO BE LOADED WITH SHOT ROCK. AS HE NEARED THE LOADER, HIS BACK TIRE ROLLED ONTO A SLIGHTLY HIGHER SPOT ON THE PIT FLOOR. THIS CAUSED HIS BED TO TILT UP HITTING LOADER ARM. HE RECEIVED STRAIN TO NECK/SHOULDER ARE. WAS 1ST AID(AND DR'S 1ST AID) UNTIL 10/13 WHEN HE BEGAN MEDICAL TREATMENT(PHYSICAL THERAPY)

EE WAS CLSING DOORS UNDER RAILCAR AND PINCHED FINGER BETWEEN METAL BAR AND CAR.

EE WAS WRAPPING UP HOSE ON PRESSURE WASHER, WHICH HE HAD TURNED OFF. HE ACCIDENTILLY SQUEEZED TRIGGER & SPRAYED HAND.

OPERATING FRONT END LOADER, MACHINE SHIFTED, ELBOW HIT AGAINST INSIDE OF CAB FRACTURING ELBOW.

ADJUSTING BELTS ON PULLY AND PINCHED RIGHT LITTLE FINGER. WENT TO DOCTOR TO GET TOPICAL OINTMENT. DID NOT KEEP FINGER CLEAN, INFECTED. HAD TO TAKE ORAL ANTIBIOTIC. NO LOST TIME, NO RESTRICTED DUTY.

STEPPED OFF LADDER OF HAUL UNIT AND FELL TWO FEET INTO THE GROUND, FRACTURE TO LEFT ANKLE, MODIFIED DUTY.
