Two west side propane vaporizer boxes and piping were not protected with jersey barriers from being struck by mobile equipment. A bobcat skid loader was back dragging sand by the east side of the dryer and struck the unprotected top cylinder of the propane piping to the vaporizer resulting in a fire.

Step in control room, misjudged footing and tripped. Miner hit EE's shin in entry of door walking into control room. Supervisor witnessed and asked if EE needed first aid. Miner declined first aid, sat down, then got up and proceeded to work. EE said EE was fine to the same Supervisor. *EE didn't report concern about injury until 3/26/18

Worker was on top of bag-house inspecting bags through open portal. Cover of portal is removable and was leaning against guardrail for portal access. Wind caused cover to fall back over portal. Worker was struck by falling cover on the right arm/elbow. Worker was unable to continue duties due to pain and limited mobility of right arm/elbow.
