July 30, 2018 TX · Metal/Non-Metal bulldozer operator, universal operator, heavy equipment operator, operating engineer OTHER
Bella Vista Rock Source · Struck by flying object

While at work checking a machine, debris blew into my eye, the material being processed was very dry and the weather was very windy. My right eye gets very irritated with wind and light. Feels like scratched every time I blink.

April 4, 2018 TX · Metal/Non-Metal bulldozer operator, universal operator, heavy equipment operator, operating engineer SLIP OR FALL OF PERSON
Bella Vista Rock Source · Fall to the walkway or working surface

Really windy. I went to remove fire extinguisher, my hard hat flew in the truck, I went and climbed in the back, as I was getting out, my left foot was on the vise base, I stepped on it and twisted my ankle, I went to fall off the bumper and put my right foot down the ground; twisted my right knee and hit my left elbow.
