Fall from ladders

Employee was changing a valve when a bolt broke causing the valve to swing and hit EE knocking EE off of the ladder. Landed on their hip on the concrete pad.

Struck by flying object

Employee had cleared a jam from the cone crusher and was making sure it was going to run correctly. While observing the cone crusher a piece broke off and became lodged then ejected striking the employee in the hard hat. Employee states that they had a severe headache, legs felt weak and they were sensitive to light. Went to emergency room and was diagnosed with a concussion.

EE was tightening the clip on EE's radio when the screwdriver slipped and punctured EE's hand.

The EE was dismounting the excavator when their finger became lodged and upon trying to free it cut the finger open.

The EE was attempting to replace the fuel filter for the excavator, when diesel fuel splashed on the employee.

The mechanic was in the process of replacing a bearing when the mechanic struck left thumb with a 4 pound hammer causing a laceration. The employee was taken to the emergency room and received stitches to thumb.

After the employee climbed down the steps to exit the cab of the Mac Dump truck, EE tripped over uneven ground and fell to the same level. The employee fell on their left shoulder injuring it.

Employee was stepping onto the second landing of stairs when coming down from the top of the silos at the dry plant, when EE's knee gave out and caused EE to fall on to the landing when EE stepped onto it. The employees doctor placed EE off of work due to EE's injury.

Employee took a step and felt a pop in foot while doing normal job at the wet plant. The employee went to the emergency room and got a splint for left foot on the date of the accident. Follow up occupational medical visit on 09/22/2021 resulted in placing the employee on restricted duty.

The employee ate flavored sunflower seeds that another employee offered EE. The employee has a food allergy, and eating the sunflowers caused EE to have a reaction which caused EE to become light headed and itchy. The employee didn't remember the ride to the hospital so we are reporting the loss of consciousness, since it happened while EE was at the mine site.

Over-exertion in lifting objects

Employee was helping to repair storm damage and replacing conveyor belt back on the conveyor system. When flipping the belt the employee's right shoulder started aching. Employee did not notify supervisor until 9/30/19 of the injury.

EE was changing bags in the baghouse, when sand/dust became lodged under EE's contacts. EE was wearing Safety Glasses at the time.

Employee was operating the 966 Loader when sand became lodged underneath employee's contacts scratching eye.

Employee was digging wet sand by hand, and needed to move a piece of pipe. EE reached down and picked up the pipe, cutting EE's hand on the edge.
