Caught in, under or between a moving and a stationary object
EE was cleaning the impactor, which held a boulder in it. As EE was cleaning it, EE slipped and EE's leg got caught in it. LOTO was in place, however the impact of EE's leg on rotor caused it to rotate and bruised employees leg.
