On 05/03/2023 IE presented an EE with work restrictions ordered by a physician for shoulder pain. EE believes that the shoulder pain was caused while using an impact wrench in early April. IE did not report the injury to the employer until 05/03/2023.

Caprock Calcium, LLC · Struck against a moving object

Driver of Mack truck and belly dump trailer traveling on private caliche road approached a sharp turn at an unsafe speed and lost control of vehicle. Trailer and cab tipped and landed on its side. Driver was fully trained on the Mack/trailer. Driver sustained a laceration on EE's forearm requiring stitches.
