Fall from machine
employee was stepping off machine and stepped wrong on the first step starting from the top, they slipped and fell on their back. Other employees helped them get up, they did get the wind knocked out of them, but felt ok. Their supervisor did ask them if they wanted to go to the emergency room and they denied medical attention.
