EMPLOYEE WAS PUTTING A STEEL PIN INTO A SWING SHEVE, IT WOULD NOT GO, SO HE PUT HIS FINGERS UNDER THE FLANGE TO TAKE IT OUT ANDTHE PIN DROPPED CATCHING HIS LEFT MIDDLE FINGER AT THE NAIL AREA.
