Employee was changing the air filter in Semi truck. Slipped off truck and fell to ground hitting EE's head

Employee was changing a part in the Vertical Shalt Conductor and the door shut on EE's finger

Index finger on right had got caught in the chute of the bagging machine and finger was cut and torn from the bone

Employee was drilling a hole in flashing on crusher and the drill slipped and drilled/cut into left hand between first and second finger in palm.

Employee was carrying an electric motor to replace a motor that was no longer working. EE felt a strain in groin area. EE reported the incident on 2/25/22 to employer when a hernia became apparent. EE has not missed any work time.

Trying to dislodge 6"" rock from 3/4"" rock bin feeder. Rock gave way and pry head in the forehead

Employee was stapling slip sheets on pallets and EE moved/lifted a pallet and strained lower back.

An employee with the help of another employee was getting a piece of sheet metal off storage rack. One employee pulled on sheet metal and other employee got little finger pinched between sheet metal. EE received 5 stitches for cut.

Employee was installing a bracket in a loader to hold a CB. Using drill and screws to put bracket into place. Screw slipped and went to left hand.
