Employee was walking stumbled and fell sideways 14 feet from a step over ladder they had traversed landing on their right side with pain on that side and shoulder 911 was called and the employee was transported to Hospital.

EE was using EE's personal knife to remove a band from a stack of bags. In cutting the strap the force used the knife to continue on toward the EE left forearm causing a laceration. EE was taken to a local urgent care facility were the wound was treated and stitches applied. EE may return to work tomorrow.

Employee was dropping off a loaded trailer in the truck parking lot. While cranking the dollies on the trailer down, a bee landed on the employee's right side. This resulted in the employee being strung by the bee and the employee having to self administer an Epipen as employee is allergic to bee venom.
While walking on an elevated walkway employee did not observe that an access opening in the grating was open and subsequently stepped through it causing a contusion to his lower leg.

Employee was operating a machine that fills polypropylene bags with pulverized limestone when his left hand got caught in one of the straps attached to the bag, as the forklift operator was backing out with the pallet that the bag was sitting on resulting in the employee's left arm being pulled causing a strain to the machine operator's left shoulder.

While replacing a brake chamber on a commercial truck, the employee unsafely attempted to disassemble the factory sealed portion of the brake chamber. The spring pressure discharged the brake chamber causing pieces of the spring to strike the employee over his left eye, causing a very deep laceration.

(Worker breached safety barrier and fell 4 feet to warehouse floor.) A roll of plastic covers was being installed with a forklift. EE was positioned inside a safety perimeter attempting to position the locks on the shaft of the roll. For whatever reason he exited the safety perimeter and fell off the elevated work area about 4 feet to the warehouse floor.
Employee was tightening straps on truck for load, hands were wet from snow, hands slipped off strap bar and bar flew up and hit him in chin. Employee was using a DOT approved strap bar.

We fill this report with prejudice and that the indicient was not reported to supervisory personnell.

Employee opened the gate to the restricted area, overrode the electrical safety switch by blocking it off and proceeded to dislodge the top sheet while the machine was operating. The top sheet dispenser returned trapping his arm and shoulder against the metal structure.

Forklift operator was putting tarp on top of loaded trailer. Tarp was heavier than usual because of rain and mud. He twisted his left wrist.
