While unloading empty glue boxes off drill, a piece of rock fell and struck him causing a laceration to his ear, requiring stitches. Rock measured 24"" x 40"" x 3"".

A non-intentional, non-injury roof fall occured at spad 2706, The fall was in the intersection and through the left break. Size of the fall measured 50' long x 4.5' - 5' thick x 7' -10' wide. Roof support was 4' fully grouted 75 grade resin bolts. No equipment was involved. Ventilation was not affected. Fall will not be cleaned up.

Draw rock fell striking employee on right arm causing a cut requiring 2 stitches.

Employee was preparing to load a used scoop tire, using pinner steel inserted in rim of tire to set tire upright when the coupler on the end of steel broke just as tire was upright, tire flipped back, employee lost balance and fell backwards, catching himself with left hand, fracturing wrist.

Employee was jacking up supply car, when jack kicked out and car fell, contacting his right foot.

Two employee's were using a block hammer and pick hammer to split a timber. The injured was holding the timber, when a sliver of metal from the hammer 1/4"" long, entered the employee's left hand.

A fall of the roof 40' long, 24' wide and 16' to 20' high occurred in the No. 2 heading covering the Joy 14-10 continuous miner JM505 on the 001 section 80' inby SS# 1847.

Employee was installing curtain board, put pressure on 18"" bolt which bent and kick around, causing the bolt plate to strike employee above the right eye, causing a laceration.

A fall occurred in the # 6 entry of Southwest Mains, 1 break inby spad # 1452. The fall goes through 6 right into 7 right breal. This is an abandoned area of the mine. The fall is approximately 10' to 15' high and 120"" long. Was discovered today. Actual date of fall is unknown.

A fall occurred in the #5 heading of the 2 Northwest Mains section at spad 1562. This is approximately 350' outby the active working section. No equipment, no ventilation disturbed, no injuries. Fall will not be cleaned.

A methane ignition was suspected to have occurred on 01/12/2007 on the active working 002 section while coal was being extracted with the Joy 14CM15 mining machine. No one was injured.

Employee stated he was unloading supplies off flat car with scoop, had hand over top of scoop canopy, raised scoop up with bucket, caught hand between scoop and mine roof.

After making a visual inspection, the employee decided to pull a piece of draw rock for the other operator. The employee was using an 8 foot long slate bar, the rock fell and the end of the slate bar caught on the T-bar, which caused the end that he was holding in his hands to strike him in the nose.

A methane ignition occurred on the active working section MMU 001, in the face of the No. 1 heading off the #2 Northwest Mains. The ignition occurred while coal was being extracted with the Joy 14CM-10A continuous mining machine. No fire needed to be extinquished. Nobody was injured.

A roof fall occurred measuring 20' X 75' X 7-8' thick at SS 1526 intersection thru right crosscut in the #4 entry. No equipment was involved, stopping repaired, no injury occurred and the fall will not be cleaned up at this time.

Employee was trying to unplug the dust hose on the roof bolter. His hand was top of the drill chuck when he raised the drill boom in order to get to the fitting on the bottom of the drill pot and caught his hand between the drill chuck and steele that was in the hole.

Employee was troubleshooting the #8 belt drive power unit. He checked a breaker that was not functioning propertly. He removed the cover on the breaker and checked the under voltage release mechanism, when a flash occurred. The employee received 2nd degree burns to both hands and minor burns to his face, neck and left ear lobe.

Employee was putting hydraulic oil in roof bolter, when he accidentally cut his right pinky finger, resulting in a laceration that required stitches.

A coal rib broke free, falling and striking employee on the back of his head, causing a laceration that required staples.

A roof fall occurred at survey station 1427 in the intake airway, roof fall discoverd by EE, size of roof fall 25' long x 25' wide x 18' high. Roof fall does not effect ventilation or travel and will not be cleaned up.

Methane ignition occurred on the 001-0 MMU in the No. 4 entry, the flames were extinguished using the water sprays on the head of the continuous miner.

Miner operatot was in the process of cleaning up the mining cut, when a methane ignition occured in the face of number 9 entry being mined.

Employee was using a rope grip hoist to make a belt splice, handle pin sheared, causing him to fall on his right knee resulting in a sprain.

A piece of draw rock fell from overhead from between the roof bolt and struck employee on the left forearm causing a laceration that required stitches.

Employee was pushing a rock off arm of atrs when his fingers were mashed between the rock and mine roof, causing fractures and lacerations to fingers on his left hand.

A ROCK FALL OCCURRED, MEASURING 20' X 20' X 5', ONE BREAK INBY SPAD 349, IN THE INTERSECTION. NO EQUIPMENT WAS INVOLVED, NO VENTILATION WAS DISTURBED, NO INJURY OCCURRED AND THE FALL WILL NOT BE CLEANED UP.

Employee was working on suction on offside of bolter when the operator side operator came around to offer assistance. The operator from the operator side leaned over to help, hitting the lever causing the head to go up, catching offside operator's fingers between the head arm and the arm going to the feed jack. He suffered lacerations and fractures to fingers on his left hand.

EMPLOYEE WAS TRAVELING FROM #4 TO #5 HEADING, WHEN HE HIT HIS FACE ON A BENT ROOF BOLT, CAUSING A LACERATION TO HIS FACE THAT REQUIRED STITCHES.

EMPLOYEE STEPPED OFF THE MAC 8 AND SAID HE EXPERIENCVED A PAIN IN HIS RIGHT KNEE AND COULD NOT BEND IT WITHOUT INCREASING THE PAIN. THE AREA WAS CLEAN AND DRY WITH MINE AROUND 60"" TO 64"" HIGH

EE was operating a unihauler when his little finger on his right hand became pinched between the machine and the coal rib, causing a lacertion.

A piece of draw rock fell from the left rib towards the operator when the drill canopy was lowered, striking his right leg causing a fracture. Employee continued to work until 5-26-05.

EE was installing the last row of bolts when a piece of draw rock fell from overhead striking the back side of both legs just above the ankles causing a fracture to his right leg.

A ROOF FALL WAS DISCOVERED THAT MEASURED 16' X 16' X 5' IN #3 PORTAL, JUST INBY THE PORTAL SHIELD. NO EQUIPMENT WAS INVOLVED, NO VENTILATION WAS DISTURBED, NO INJURY OCCURED & THE FALL WILL BE CLEANED UP.

Employee put his foot under head of roof bolter & the head went down on his foot.

Laying cable on rid. Rock fell between rib & bolts. Hit employee on back.

WALKING ACROSS SECTION HIT HEAD ON TOP. HURT NECK. MAN CAME BACK TO WORK, WENT BACK TO DOCTOR ON 27 JAN 2004.

MINER WAS CUTTING COAL IN THE NO. 2 HEADING WHEN HE THOUGHT HE HAD AN IGNITION. NO DAMAGE TO EQUIPMENT OR EMPLOYEES.

MINER WAS CUTTING COAL IN NO 6 RT ENTRY WHEN HE WAS THROUGH HE HAD AN IGNITION. NO DAMAGE DONE, NO ONE HURT.

EE WAS HELPING ON BELT WHEN SOMETHING POP IN HIP

FALL FOUND BY FOREMAN - #4 ENTRY - 1 BRK INBY #800 - NEUTRAL - FALL APP 20' X 20' X 6'. NO BLOCKAGE OF VENT, NO EQUIP INVOLVED, NO BLOCKAGE OF ESCAPEWAY, NO INJURIES. 7400' OUTBY WORKING FACE. BREAKERED OFF AND DANGERED. WILL NOT BE CLEANED UP. PLOTTED ON MINE MAP. MINE IN A NON PRODUCING STATUS - IDLE.

A ROOF FALL WAS DISCOVERED IN THE #2 ENTRY OF SW MAINS 10' OUTBY S.S. 739 & 600' OUTBY FACE. THE FALL MEASURED UP TO 12' THICK, 50' LONG & 18' WIDE. NO INJURIES, NO BLOCKAGE OF VENTILATION OR ESCAPEWAY, NO EQUIPMENT INVOLVED. WILL NOT BE CLEANED UP - TIMBERED OFF & DANGERED.

FALL DISCOVERED 9-17-02 AROUND 9:30 AM FALL 20'WIDE 10' LONG 4' THICK-NO INJURED NO EQUIPMENT INVOLVED NO BLOCKAGE OF S SCAPEWAY OR VENT. WILL NOT BE CLEANED UP TIMBERED OFF AND DANGERED.

EE STATED HE WAS CHECKING PERMISSIBILITY ON 3-WHEELER PANEL BOARD-WHEN CHECKING BOTTOM OF PANEL-HE STRUCK HIS HAND AGAINST A PIECE OF GLASS.

EMPLOYEE STATED THAT HE HAD STARTED A ROOF BOLT IN HOLE AND WAS STRAIGHTNENING THE BOLT WHEN HE FELT PAIN IN HIS ABDOMEN.

EMPLOYEE WAS ASSISTING TO ADVANCE THE CONVEYOR BELT AND WHILE HANDLING AND LIFTING THE BELT STRUCTURE HE FELT PAIN IN HIS GROIN AREA. THE VICTIM WAS DIAGNOSED AS HAVING A STRAIN ON DEC 6, 2001 .

AN UNEXPLAINED FALL OF ROOF MEASURING APPROX. 11' WIDE X 4' HIGH AND 12'LONG FELL ONTO RIPPERS OF MACHINE AS THEY WERE TRAMMING FROM THE FINISHED CUT OF THE #31 LIFT OF A 5 CUT PARTIAL PILLAR RECOVERY PLAN FOULING MACHINE. MACHINE WAS DISLODGED WITH ASSISTANCE OF ATTACHED UNIT. AREA WAS TIMBERED OFF AND MINING WAS CONTINUED.

EE WAS TRANSPORTING A PIECE OF SHEET METLA IN THE OUTBY END OF A TRACK OPERATED PERSONNEL CARRIER. THE METAL VIBRATED LOOSE, STRUCK COAL RIB AT A CLOSE CLEARANCE AREA, CAME AROUND AND STRUCK V ICTIM.

EE WAS TRYING TO START A DRILL HOLE IN AN AREA OF SLOPING ROOF WHEN A PIECE OF ROCK MEASURING APPROX 18"" LONG X 12"" WIDE X 8"" THICK FELL FROM ROOF MASHING HAND BETWEEN ROCK AND DRILL HEAD.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED MEASURING 5 1/2'H X 45'L X 20'W. ABOUT 4000' OUTBY FACE 1 BREAK INBY SURVEY SPAD #439 IN LEFT RETURN AIRWAYS. AREA SUPPORTED BY 4' FULLY GROUTED RODS.

EE WAS IN THE PROCESS OF MAKING A PRE-SHIFT ELECTRICAL EXAMINATION. HE GOT OUT OF PERSONNEL CARRIER AND AS HE BENT OVER TO TRAVEL THROUGH A 30"" X 30"" MANDOOR HE FELT PAIN IN HIS LOWER BACK. TH IS EE WILL BEGIN RESTRICTED WORK DUTIES 2-1-01.

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE WORKED OUT #2 PANEL LT OFF N. WEST MAINS. FALL MEASURED APPROX 70' LONG 20' WIDE 8' HIGH. AREA HAD BEEN SUPPORTED WITH 4' AND 5' RESIN RODS. THE FALL AREA HAD BEEN EXPERIENCING ADVERSE ROOD CONDITIONS DURING DEVELOPMENT.

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE WORKED OUT #2 PANEL LT OFF N WEST MAINS. FALL MEASURED APPROX 70' LONG X 20' WIDE X 8' HIGH. AREA HAD BEEN SUPPORTED WITH 4 & 5' RESIN RODS. THE FALL AREA WAS EXPERIENCING ADVERSE ROOF CONDITIONS DURING DEVELOPEMENT.

EE WAS CUTTING SHIPPING STRAPS FROM A NEW CONVEYOR CHAIN WITH A KNIFE. THE KNIFE SLIPPED CUTTING LT INDEX FINGER.

AN UNINTENTIONAL ROOF FALL OCCURRED IN THE #5 ENTRY OF OLD 3 MAINS AT SURVEY SPAD #U446. THE FALL MEASURED APPROX 40'LONG, 20'WIDE, AND 6.5'HIGH. FALL AREA HAD BEEN SUPPORTED WITH 5' RESIN ROD S. NO EQUIPMENT, VENTILATION OR TRAVEL WAS AFFECTED. AREA APPROACHED WERE TIMBERED AND WILL NOT BE CLEANED UP.

EE WAS CARRYING A BAG OF ROCKDUST AND SLIPPED IN AN OFFSET IN MINE FLOOR. HE WAS CARRYING ROCKDUST BAGS TO LAY AGAINST TRACK RAIL FOR SCOOP TO CROSS TRACKS.

AN UNPLANNED ROOF FAL WAS DISCOVERED AT 12:45AM IN #3 INTERSECTION AT SURVEY SPAD #262. FALL MEASURED APPROX. 18' WIDE, 4' HIGH, 55' LONG. CUTTING CONTROL CABLES TO CONTINUOUS MINER & MOBLE ROOF SUPPORTS. NO PERSONS OR EQUIPMENT WERE CAUGHT. CABLES WERE REROUTED AND SPLICED AND EQUIPMENT MOVED FROM EFFECTED AREA.

AN UNINTENTIONAL ROOF FALL OCCURRED DURING RETREAT MINING. AS CONT. MINER WAS BACKING FROM CUT THE ROOF MEASURING APPROX. 15' W X 20' L X 3 1/2'H, FELL ONTO MINER. A RETRIEVER WAS USED TO REMO VE MACHINE FROM FALL AREA, NO INJURIES OR PROPERTY DAMAGE OCCURRED. AREA WAS TIMBERED OFF AND MINING RESUMED ON NEXT PILLAR.

EE WAS MAKING A BELT MINE AND WHILE CARRYING A PIECE OF BELT STRUCTURE (STAND) HE TRIPPED ON A PIECE OF COAL LAYING IN WALKWAY AND DROPPED STAND ONTO HIS FOOT.

EE WAS STANDING AND STARTED TO TURN AND FELL. WHILE FALLING EXTENDED HIS RIGHT ARM OUT TO PREVENT FALL. HE FELL ON HIS RIGHT ARM AND CAUSED A CLOSED FRACTURE TO ARM.
