Struck by falling object

While unloading empty glue boxes off drill, a piece of rock fell and struck him causing a laceration to his ear, requiring stitches. Rock measured 24"" x 40"" x 3"".

Accident type, without injuries

A non-intentional, non-injury roof fall occured at spad 2706, The fall was in the intersection and through the left break. Size of the fall measured 50' long x 4.5' - 5' thick x 7' -10' wide. Roof support was 4' fully grouted 75 grade resin bolts. No equipment was involved. Ventilation was not affected. Fall will not be cleaned up.

Draw rock fell striking employee on right arm causing a cut requiring 2 stitches.

Fall to the walkway or working surface

Employee was preparing to load a used scoop tire, using pinner steel inserted in rim of tire to set tire upright when the coupler on the end of steel broke just as tire was upright, tire flipped back, employee lost balance and fell backwards, catching himself with left hand, fracturing wrist.

Employee was jacking up supply car, when jack kicked out and car fell, contacting his right foot.

Two employee's were using a block hammer and pick hammer to split a timber. The injured was holding the timber, when a sliver of metal from the hammer 1/4"" long, entered the employee's left hand.

A fall of the roof 40' long, 24' wide and 16' to 20' high occurred in the No. 2 heading covering the Joy 14-10 continuous miner JM505 on the 001 section 80' inby SS# 1847.

Employee was installing curtain board, put pressure on 18"" bolt which bent and kick around, causing the bolt plate to strike employee above the right eye, causing a laceration.

A fall occurred in the # 6 entry of Southwest Mains, 1 break inby spad # 1452. The fall goes through 6 right into 7 right breal. This is an abandoned area of the mine. The fall is approximately 10' to 15' high and 120"" long. Was discovered today. Actual date of fall is unknown.

A fall occurred in the #5 heading of the 2 Northwest Mains section at spad 1562. This is approximately 350' outby the active working section. No equipment, no ventilation disturbed, no injuries. Fall will not be cleaned.

A methane ignition was suspected to have occurred on 01/12/2007 on the active working 002 section while coal was being extracted with the Joy 14CM15 mining machine. No one was injured.

Employee stated he was unloading supplies off flat car with scoop, had hand over top of scoop canopy, raised scoop up with bucket, caught hand between scoop and mine roof.

After making a visual inspection, the employee decided to pull a piece of draw rock for the other operator. The employee was using an 8 foot long slate bar, the rock fell and the end of the slate bar caught on the T-bar, which caused the end that he was holding in his hands to strike him in the nose.

A methane ignition occurred on the active working section MMU 001, in the face of the No. 1 heading off the #2 Northwest Mains. The ignition occurred while coal was being extracted with the Joy 14CM-10A continuous mining machine. No fire needed to be extinquished. Nobody was injured.

A roof fall occurred measuring 20' X 75' X 7-8' thick at SS 1526 intersection thru right crosscut in the #4 entry. No equipment was involved, stopping repaired, no injury occurred and the fall will not be cleaned up at this time.

Employee was trying to unplug the dust hose on the roof bolter. His hand was top of the drill chuck when he raised the drill boom in order to get to the fitting on the bottom of the drill pot and caught his hand between the drill chuck and steele that was in the hole.

Flash burns (electric)

Employee was troubleshooting the #8 belt drive power unit. He checked a breaker that was not functioning propertly. He removed the cover on the breaker and checked the under voltage release mechanism, when a flash occurred. The employee received 2nd degree burns to both hands and minor burns to his face, neck and left ear lobe.

Employee was putting hydraulic oil in roof bolter, when he accidentally cut his right pinky finger, resulting in a laceration that required stitches.

Struck by falling object

A coal rib broke free, falling and striking employee on the back of his head, causing a laceration that required staples.

A roof fall occurred at survey station 1427 in the intake airway, roof fall discoverd by EE, size of roof fall 25' long x 25' wide x 18' high. Roof fall does not effect ventilation or travel and will not be cleaned up.

Employee was using a rope grip hoist to make a belt splice, handle pin sheared, causing him to fall on his right knee resulting in a sprain.

A piece of draw rock fell from overhead from between the roof bolt and struck employee on the left forearm causing a laceration that required stitches.

Employee was pushing a rock off arm of atrs when his fingers were mashed between the rock and mine roof, causing fractures and lacerations to fingers on his left hand.

A ROCK FALL OCCURRED, MEASURING 20' X 20' X 5', ONE BREAK INBY SPAD 349, IN THE INTERSECTION. NO EQUIPMENT WAS INVOLVED, NO VENTILATION WAS DISTURBED, NO INJURY OCCURRED AND THE FALL WILL NOT BE CLEANED UP.

Employee was working on suction on offside of bolter when the operator side operator came around to offer assistance. The operator from the operator side leaned over to help, hitting the lever causing the head to go up, catching offside operator's fingers between the head arm and the arm going to the feed jack. He suffered lacerations and fractures to fingers on his left hand.

EMPLOYEE WAS TRAVELING FROM #4 TO #5 HEADING, WHEN HE HIT HIS FACE ON A BENT ROOF BOLT, CAUSING A LACERATION TO HIS FACE THAT REQUIRED STITCHES.
