EE WAS OPERATING DOZER DOWN SLOPE AND RAN OVER ROCK WHICH JARRED BACK, FELT PAIN IN BACK AND WAS TAKEN HOME. LATER THAT AFTERNOON WIFE TOOK HIM TO DR. RETURNED TO WORK ON NEXT REGULAR SHIFT 9- 10-95 ON RESTRICTED WORK ACTITIVTY.

ON 5 -1,02, AND 03 OFF DAYS. FIRST DAY MISSED 5-6-95. RETURNED TO WORK 5-5-95, OFF 5,6,7, AND 8-95. RETURNED TO WORK ON 5-13-95.

EMPLOYEE WAS USING A HAMMER TO INSTALL BEARINGS ON DOZER WHEN A PIECE OF METAL DISLODGED AND STRUCK HIM ON HIS RIGHT HAND. EMPLOYEE DID NOT BELIEVE THE METAL HAD ENTERED HIS HAND, HOWEVER, ON 10-31-94 EMPLOYEE REPORTED HE COULD FEEL THE METAL IN HIS HAND. EMPLOYEE WAS TAKEN TO THE DR.AND THE METAL WAS REMOVED, REQUIRING TWO STITCHES.
