EMPLOYEE STATED HE FELT A JERK AND PAIN IN HIS SHOU LDER BUT DID NOT WANT TO GO TO THE DR. EMPLOYEE WENT TO THE DR ON 6-2-94 AND WAS OFF WORK UNTIL6-11-94. EMPLOYEE RETURNED TO DR WITH PERSISTENT PAIN ANAD HAS BEEN OFF WORK SINCE 6-15-94. EM
