Mashed or cut 2nd finger when small beam 4' long caught his finger between beam and loader bucket as he was loading it into dipper.
An 18 year old young man was working on the 15 foot level work deck assisting with rebar and concrete form at top of shaft. Someone dropped a hammer down to next deck @ 25'. He slipped off the manway compartment ladder and fell down 13 feet on the manway and then rolled off a platform down another 6' - 9".
I had trammed coal load of rock (800#) from work place to shaft with employee. Went through hook up to pull bucket from mine. He filled second bucket, trammed part way to shaft (130'), got hung up due to friction on a curve and either jammed finger or pinched it with the pail.
