Struck by... (Not Elsewhere Classified)

Employee was sent up to shop area to remove a 1 1/2"" nipple out of a 1 1/2"" elbow, using a vise and two pipe wrenches. He allowed one hand to be too close to the 2nd wrench and the wrench slipped on the nipple hitting his upper fingers with the handle end. He used a smaller wrench when 2 large wrenches were available

Mashed or cut 2nd finger when small beam 4' long caught his finger between beam and loader bucket as he was loading it into dipper.

Fall from ladders

An 18 year old young man was working on the 15 foot level work deck assisting with rebar and concrete form at top of shaft. Someone dropped a hammer down to next deck @ 25'. He slipped off the manway compartment ladder and fell down 13 feet on the manway and then rolled off a platform down another 6' - 9"".

Struck against stationary object

I had trammed coal load of rock (800#) from work place to shaft with employee. Went through hook up to pull bucket from mine. He filled second bucket, trammed part way to shaft (130'), got hung up due to friction on a curve and either jammed finger or pinched it with the pail.
