Miner was checking oil on the D8 Bulldozer and they slipped on the tracks. Their foot went between the tracks and cab, and their ribs hit against the grab handle. They injured their ribs. 2 weeks later on 12/1/25, they sought medical care and were prescribed a muscle relaxant.

Employee had gotten off of forklift to straighten a bulk bag. Employee stepped on partial pallet (uneven surface) and pulled on bag position & force applied caused strain in employees back. Employee felt fine until next day(s) Sat/Sunday & reported injury Monday 4/14/14.

EE was packing 50 lb bags at parking station a bag blew up, causing dirt to get in his right eye. EE was wearing safety glasses at the time of the accident.
ANOTHER EMPLOYEE HAD GOTTEN A LIFT TRUCK STUCK AND SECURED INJURED EMPLOYEE'S ASSISTANCE IN GETTING IT "UNSTUCK". THEY ATTACHED A TOW CHAIN AND ATTEMPTED TO PULL OUT THE OTHER LIFT TRUCK. ON THE 4-5 ATTEMPT, A LINK IN THE CHAIN BROKE AND THE CHAIN WHIPPED AND HIT EE IN THE FACE.
EMPLOYEE WAS REPLACING #2 MICROSIZER MOTOR. HE HAD LOWERED OLD UNIT THROUGH A HOLE CREATED IN THE FLOOR FOR THIS PURPOSE. HE LEFT THE AREA TO SECURE PARTS/TOOLS, REENTERED THE AREA TO GET HIS TOOLS AND STEPPED THROUGH THE HOLE IN THE FLOOR.

EE WAS PACKING 50 LB BAGS. HE WAS CLOSE TO EMPTYING THE BIN WHEN A BAG BLEW UP ON #3 PACKER. HE WAS PATTING THE BAG AT THE TIME. WHEN IT BLEW UP IT THREW PRODUCT INTO HIS LEFT EYE. EE WAS WEARING SAFETY GLASSES AT THE TIME. HE WAS TAKEN TO E.R. WHERE THEY IRRIGATED EYE AND RELEASED HIM.

THE EE WAS REMOVING A TAG TRAP FROM A CONVEYING LINE. WHEN THE TRAP WAS FREE IT BEGAN TO FALL OFF LADDER APPROX. 3'OFF THE GROUND. THE EE GRABBED IT TO PREVENT IT FROM FALLING, IT JERKED HIS ARM CAUSING HIS SHOULDER TO "POP".
THE EE'S ANNUAL AUDIOGRAM SHOWED A SIGNIFICANT CHANGE IN HEARING. SPEECH RECEPTION THRESHOLDS WERE 25 DB RIGHT EAR AND 10 DB LEFT EAR. SPEECH DISCRIMINATION SCORES WERE 100% RIGHT AND 96% LEFT . MILD TO MODERATE SENSORINEURAL HEARING LOSS LEFT. MODERATE SENSORINEURAL HEARING LOSS RIGHT.

EE WAS ON HER FORKLIFT EXITING WAREHOUSE. ANOTHER EE WAS ENTERING WAREHOUSE.EE STOPPED TO LET EE PASS. EE MISJUDGED THE TURN AND BUMPED INJURED'S BACK TIRE. THE ENERGY TRANSFERED FROM HER REAR WHEELS(STEERERS) TO STEERING WHEEL TURNING IT ABRUPTLY. KNOB ON WHEEL STRUCK INJURDED'S RIGHT HAND.

WHILE DRIVING A SPROCKETT OFF A SHAFT WITH A HAMMER THE EE WAS HOLDING THE SPROCKET WITH HIS LIEFT HAND & SWINGING THE HAMMER WITH HIS RIGHT HAND. ON A BACK SWING, THE HAMMER (32 OZ BALL PEEN) HIT OFF THE CONVEYOR STRUCTURE CHANGING THE COURSE OF TRAVEL WHICH CAUSED HIS FINGER TO BE SMASHED
HE HAD JUST BEGUN HIS SHIFT. HE MADE HIS WAY TO TRUCK RAMP AND BEGAN INSPECTION. HE WAS UNAWARE THAT GRATING WAS MOVED IN SOME WAY, CREATING A GAP BETWEEN PIECES. HE APPARENTLY SIDE-STEPPED IN TO VOID AND CAUGHT FOOT IN GRATING CAUSING HIM TO TRIP AND FALL. HE LANDED ON GRATING STRIKING HIS LEFT HAND TO BREAK FALL.
THE EE WAS USING A PRY BAR TO ALIGN A WEAR LINER IN A MILL CASTING. THE BAR SLIPPED WHICH RESULTED IN HIS LEFT RING FINGER BEING SMASHED BETWEEN THE PRY BAR AND THE MILL CASTING.
EE WAS CLIMBING DOWN FROM THE TOP OF THE HAUL TRUCK WHEN A FOREIGN BODY BLEW INTO HIS RT EYE. THE FOREIGN BODY CAUSED IRRITATION AND TEMPORARILY BLURRED VISION IN THAT EYE.

EMPLOYEE WAS LOOKING FOR A HYDRAULIC LEAR IN THE GOOSE NECK OF A 631 SCRAPER. HE REQUESTED THE ASSISTANCE OF A FELLOW EMPLOYEE TO MOVE THE CROWD WHILE HE OBSERVED. EMPLOYEE HAD LOWERED THE CAN COMPLETELY, THE DOOR WAS OPEN. HE STUCK HIS HEADUNDER THE ARM THAT CONTROLS THE DOOR TO LOOK FOR LEAKS. THE OTHER EMPLOYEE LOWERED THE DOOR TO BETTER OBSERVE,THE DOOR CLOSED...(SEE REPORT)
EE WAS CLEARING A JAM-UP IN THE PACKAGING LINE, WHENTHE AUTOMATIC EQUIPMENT STARTED. HIS FOOT WHICH WAS ON SOME FRAME WORK, SLIPPED GETTING CAUGHT ON A CONVEYOR BELT, WHICH PULLED IT BELOW A R ETURN IDLER BRUISING THE ANKLE AND LEG UP PAST HIS KNEE.
EE STATED THAT HE WAS IN A HURRY DOWN SOME STAIRS AT OUR TRUCK RAMP UNLOADING STATION WHEN HE STUMBLED & FELL 3/4 OF THE WAY DOWN THE STAIRS (EE SAID HE USED HANDRAIL).
EE WAS PERFORMING PREOPERATION INSPECTION. HE WAS RETURNING TO THE CAB BY WAY OF THE FENDER WHEN HE SLIPPED ON THE SMOOTH SURFACE. HE SPRAINED HIS LEFT ANKLE AGAINST A HAND RAIL.
