Crusher operator received a deep laceration from a metal plate on their left, inside forearm while routine cleaning. Operator was transported by EMTs to hospital and received stitches.

Stepped on rock and twisted left knee. He was stepping off the loader.

While working on a maintenance item the item fell. EE reached for the item and this is when the torch burned his left arm.

The cutting edge was being removed from the loader bucket. The impact wrench twisted the bolt, which was being held by his hand. The sharp edge of the bolt cut his hand - requiring stitches and glue.

Struck by flying object

While removing the cutting edge off the loader bucket, the cutting edge fell to the floor and came in contact with a bolt which blew up and hit employee in the face cutting his lip. There was support under the bucket as well as the cutting edge.

When disposable box cutter handle broke it cut through leather gloves & right hand.

When installing chain onto sprocket, release of stored energy from bound-up chain caught middle finger on right hand. His reaction of pulling hand away tore off finger nail.

While ascending ladder on railcar, the safety harness/lanyard being worn swung around his left side and hit him in the mouth which broke off his capped tooth.
