Lane Mtn Silica Quarry & Mill: 361 MSHA citations Metal/Non-Metal

Crusher operator received a deep laceration from a metal plate on their left, inside forearm while routine cleaning. Operator was transported by EMTs to hospital and received stitches.

Employee wearing a welding hood while grinding metal, received metal flakes onto eye.

Exposed to noise in work area(s). STS reported after annual audiometric testing.

Employee's age and health. Pain was immediate and continuous.

Employee's ring caught by shoekicker when slowing down rotar in motion.

Chemical room was being washed with water from hose. The floor was sanded but ramp was not. The right foot slipped on ice while the left foot was stable on the sanded floor. EE did the splits.

Employee was replacing worn out return roller. Item fell and the sharp edge cut through glove.

Truck driver struck by falling rock.

While exiting truck, slipped on step & pulled muscle in lower back. *Employee did not report injury - notified us 9/16/19.

The loader made a sudden stop after EE hit a sub-terrain rock. EE's body was jarred from the incident.

While removing a worn plate ee lacerated finger. EE was holding a sharp spot on the metal. Also, ee was wearing leather gloves.

Employee stained back while lifting a corner of 4X8 sheet of metal.

Lifting and moving a large sheet of metal. Strained back and right shoulder.

Strained lower back while lifting & opening a frozen rail car lid.

Stepped on rock and twisted left knee. He was stepping off the loader.

While working on a maintenance item the item fell. EE reached for the item and this is when the torch burned his left arm.

The cutting edge was being removed from the loader bucket. The impact wrench twisted the bolt, which was being held by his hand. The sharp edge of the bolt cut his hand - requiring stitches and glue.

While removing the cutting edge off the loader bucket, the cutting edge fell to the floor and came in contact with a bolt which blew up and hit employee in the face cutting his lip. There was support under the bucket as well as the cutting edge.

EE was walking near the maintenance shop when he slipped and fell on the ice, which wasn't visible with snow cover.

When disposable box cutter handle broke it cut through leather gloves & right hand.

When installing chain onto sprocket, release of stored energy from bound-up chain caught middle finger on right hand. His reaction of pulling hand away tore off finger nail.

Climbed down from truck and stepped on rock, twisting right leg. We were not told he had gone to Doctor. First knowledge of incident 6/15/12.

While ascending ladder on railcar, the safety harness/lanyard being worn swung around his left side and hit him in the mouth which broke off his capped tooth.

Employee was wearing welding helmet. He was welding gripper to drill then metal piece hit right eye.

He was barring and jack hammering frozen sand chunks. Claim original injury 2/27/11 but did not go to doctor til 4/1/11.

He was disconnecting a rubber hose with a utility knife when he accidentally cut the top of his left hand.

Slipped on ice while getting into pick up, he grabbed door jam with arm and hands. Hurt his elbow. Slipped on ice 11/27/10, went to doctor 12/8/2010.

Cut left ring finger on sharp edge of square tube iron while putting away inventory.

Airborne particle entered and scratched right eye

EE was striking kicker with hammer when metal dislodged from kicker, passed through his glove, then imbedded into his left hand.

Cut right index finger with knife while retrieving falling board.

Left eye injury from using grinder.

Hurt his back while changing a pump.

Employee of Lane MTN. slipped on ice and fell breaking bones/dislocating ankle.

Exposed to noise in work area throughout 16 years.

Changing drill stems on drill over the course of many years with this company as well as his own.

Employee slipped on ice climbing onto loader-fell with fist hitting his right cage.

SILICOSIS.

HE WAS CLEANING THE CONVEYOR BELT, TURNING IT BYHAND, WHEN HIS FINGERS WAS CAUGHT IN THE PULLEY.

HE WAS COMING DOWN SOME STEPS, WHEN HE MISSED ONE AND TRIED TO CATCH HIMSELF AND IT TWISTED HIS BACK.

HE WAS KNEELING DOWN TO REPAIR THE GEAR BOX ON THE ELEVATOR, WHEN THE GEAR BOX BROKE LOOSE, HITTING HIM.

JACK SLIPPED BACKWARDS. JACK HANDLE CAUGHT FINGER BETWEEN HANDLE AND WORK BENCH.

HE WAS INSTALLING ANEW SCREEN ON CRUSHER AND REMODELING FEED SCHUTE TO CONE, WHEN HE SLIPPED AND FELL APPROX. 3 FEET TO GROUND.

USING A CUTTING TORCH,IT BLEW UP BURNING RIGHT HAND.

WHILE BACKING DOWN RAMP, HE HIT A BUMP, HURTING HIS BACK NECK AND SHOULDERS.

HE WAS PRYING ON A ROCK WITH A BAR WHEN BAR SLIPPE D HITTING HIM ON THE HEAD

HE JUMPED FROM THE BALL MILL INTO A PILE OF LOOSE SAND, ABOUT A 3' DROP. THE JUMP CAUSED BACK PAIN. WILL BE SCHEDULED FOR SURGERY ON OCT. 5, 1988.

HE WAS LIFTING 100LBS SACKS WHEN HE SPRAINED HIS BACK.

HE WAS PUTTING A MOP HEAD ON TOP OF A MOTOR AND HE GOT HIS HAND CAUGHT INTO ONE OF THE PULLEYS AND IT TORE OFF PART OF THE ENDOF THE MIDDLE FINGER
