Spraying off tracks of EX800 when hose slipped from his hands. The nozzle swung from the water pressure and struck his left ankle.

Griding weld on bed of Euclid haul truck with a 9"" grinder. Grinder slipped/got away from him & fell hitting him in the left forearm causing a cut that required stitches.

DROVE A CRANE ONTO THE LOWBOY TRUCK TO TRANSPORT. INJURED STEPPED OUT OF CAB ONTO THE GROUND, FELT SOME PAIN LATER THAT EVENING TO RIGHT UPPER THIGH. BECAME INCREASINGLY WORSE.
