Miles Sand & Gravel Company · Struck by flying object

Coworker was using a bar to loosen a piece of stuck material on the floating conveyor. When they struck the material with the bar a small piece ejected and embedded into employee's left forearm. Will need surgery to be removed - pending!

Employee fell from ladder while performing maintenance activities. The fall resulted in a ruptured disc in EE's back which required surgery.

Miner was climbing into the cab of a front end loader and missed a step. EE stated this caused injury to the back. After the shift EE was seen by the Chiropractic Doctor, who excused EE from work, and released EE to full duty without limitations on 9/19/2016.
