EMPLOYEE REPORTED TO HR THAT HE HAD BACK PAIN THAT WAS INTERUPTING HIS SLEEP AND THAT THE PAIN MAY BE CAUSED BY THE LOADER. INITIALLY THIS WAS THOUGHT TO BE A FLARE UP FROM A PREVIOUS CLAIM. RECENTLY IT WAS DECIDED THAT A NEW INJURY DATE WOULD BE ASSIGNED.

EMPLOYEE WAS ON TOP OF THE ENGINE COMPARTMENT OF HIS BACKHOE TO CHECK THE OIL. WHEN HE ATTEMPTED THE LIFT THE COMPARTMENT DOOR HIS HAND SLIPPED FROM THE HAND HOLD AND HE LOST HIS BALANCE. TO AVOID FALLING BACKWARDS HE TURNED AROUND AND JUMPED. THE JUMP RESULTED IN A BREAK OF THE LOWER TIBIA.

EMPLOYE CLIMBED UP ON CONVEYOR TO PLACE A SHIELD ON THE PEP SCREEN TO CONTROL FUGITIVE MATERIAL. HE INDICATED HE TRIPPED ON A GUARD, FALLING TO THE GROUND AND LANDED ON BOTH HANDS AND ONE KNEE CAUSING THE WRIST INJURY.

AFTER CLEANING MATERIAL OFF EQUIPMENT EE STEPPED OFF THE EQUIPMENT AND ONTO A CHUNK OF MATERIAL. CAUSING THE TWIST TO THE LEG AND KNEE. HE CONTINUED TO WORK UNTIL THE END OF 12-21-89. 12-22-89 WAS HIS FIRST FULL DAY OFF WORK.

I.NAME NOTICED THE CONVEYOR BELT HAD STOPPED AND WHILE HE WENT TO THE CONTROLS TO SHUT THE CONVEYOR OFF HE NOTICED THE BELT HAD SLIPPED FROM THE PULLEY. WHEN I.NAME ATTEMPTED TO PUT THE BELTS BACK ONTO THE PULLEY THE DRIVE MOTOR STARTED RUNNING. HIS FINGER APPARENTLY GOT PINCHED BETWEEN THE PULLEY AND BELT. THE DRIVE MOTOR APPARENTLY DID NOT SHUT OFF AT THE SWITCH.
