Fell off ladder, lost balance

EE hopped out of the loader and fell on the ground hitting his elbow on a piece of stone. He went to the emergency room - had x-rays taken which showed a previous bone spur had broken off. He went to the emergency room 5 days after on 5/21/14. Did not leave work on the day of the injury.

Employee was adding oil in hydraulic tank. Employee went up and down four steps 26 times. Employee slipped on bottom step. This caused a sprained knee and stretched ligaments.

Employee was loading stones on a pallet and some stones fell off on his foot. Employee has bruised foot.

Got foreign material in left eye.

Quarry worker slipped on gravel.

Repetition of lifting and bending for stone.

The EE was loading stone onto pallets & a stone slipped & fell on right foot.

The employee was lifting stone pavers & placing on pallets. He noted pain to upper back.

EE WAS MOVING CRAWLER IN THE QUARRY. TURNED CRAWLER THE WRONG WAY AND GOT PINNED BETWEEN A PALLET OF STONE & THE CRAWLER.

HE WAS THROWING STONE INTO TUMBLER MISSED THE HOLE HIT LEFT HAND OF EDGE OF TUMBLER.

WHILE OPERATING LOADER FELT PAIN IN LOWER BACK AND GOT WORSE.

STONE FELL ON HAND- HURT HIS LT. WRIST.
