EE hopped out of the loader and fell on the ground hitting his elbow on a piece of stone. He went to the emergency room - had x-rays taken which showed a previous bone spur had broken off. He went to the emergency room 5 days after on 5/21/14. Did not leave work on the day of the injury.

Employee was adding oil in hydraulic tank. Employee went up and down four steps 26 times. Employee slipped on bottom step. This caused a sprained knee and stretched ligaments.
Employee was loading stones on a pallet and some stones fell off on his foot. Employee has bruised foot.
